Showing all 18Employee was stepping off a ladder and stepped on a piece of wood and twisted their knee.
Employee changing out the access door hinges on the loader. Had changed out one and forgot to put the door lock pin in place to hold the door. Went to change the second hinge and the door came down on their head.
Pipe wrench slipped off of pully wheel and pinned left foot with pipe wrench handle.
After safety check, operator turned the screening plant motor on to warm up. After 2-4 minutes of the motor running, the operator noticed a white vapor coming from the hydraulic pump. Operator went to shut the screening plant down. When the operator attempted to turn the key off (shutting down the plant) the hose came off the pump, resulting in hydraulic oil spraying the operator.
EE 1 & EE 2 skid steer down with engine compartment door open. EE 1 went to get headlamp, to check tension on alternator belt. EE 1 returned did not see EE 2, EE 1 believed EE 2 was in pick-up. EE 1 looked in rear windshield of skid steer, did not see EE 2. EE1 touched the alternator belt at the same time EE 2 tried to start skid steer. EE 1 felt the belt move and pulled hand back.
Employee was pulling apart a 4 inch hose on the wash plant, when the hose broke free, the cam lock struck the employee on the mouth.
Employee walking down ramp from Jaw at aggregate plant. Fell to the ground, twisting left leg during the fall. Ground was wet at time of the fall.
Employee was welding and grinding, believes they got something in their eye. They flushed the eye.

Employee was on knees shoveling under a screen. Operator was throwing material to left side, felt a pop then a burning feeling in right shoulder. Sprain to right Rotator Cuff, restricted duty and prescription.
Employee was handing a piece of metal up to another employee, the piece of metal slipped and fell back down on employees arm.

Injured employee was climbing down a ladder with a bucket of tools in one hand, he missed the bottom rung on the ladder and fell backwards, landing on a 6x6 piece of blocking.
Employee bent over to pick up board from ground. Board was frozen to ground, resulting in strain to employee's back.
EMPLOYEE WAS PUNCHING HOLES IN CONVEYOR BELT ON GROUND WITH A HOLE PUNCH & A 2 1/2 LB. HAMMER. EE MISSED AND STRUCK HIS RT. INDEX FINGER WITH THE HAMMER, REQUIRING 6 STITCHES TO THE LACERATION.
