Employee changing out the access door hinges on the loader. Had changed out one and forgot to put the door lock pin in place to hold the door. Went to change the second hinge and the door came down on their head.

After safety check, operator turned the screening plant motor on to warm up. After 2-4 minutes of the motor running, the operator noticed a white vapor coming from the hydraulic pump. Operator went to shut the screening plant down. When the operator attempted to turn the key off (shutting down the plant) the hose came off the pump, resulting in hydraulic oil spraying the operator.

EE 1 & EE 2 skid steer down with engine compartment door open. EE 1 went to get headlamp, to check tension on alternator belt. EE 1 returned did not see EE 2, EE 1 believed EE 2 was in pick-up. EE 1 looked in rear windshield of skid steer, did not see EE 2. EE1 touched the alternator belt at the same time EE 2 tried to start skid steer. EE 1 felt the belt move and pulled hand back.
