Employee was helping another employee lift a plate of steel. The weight shifted and pulled the employee,s shoulder. He reported the incident to his Supervisor. It was handled as First Aid Only. He was notified on January 8 that he would be laid off on March 9. On January 9 he went to the doctor and was put on light duty.

Ee was using a flubber (air lance) to loosen compacted material in a cement silo. While lifting on the flubber he felt a pain in his shoulder. This accident was reported on 9/4. It was not until Friday, 9/18 that the employee requested to be seen by a doctor. Report filed on 9/24, three working days later.

On 8/29/2008, the employee was swinging a 10 pound sledge hammer against the side of a rail car to unload stuck limerock roadbase and suffered a strain in his R shoulder. The shoulder was sore for a couple of weeks and did not improve. On 9/15/2008 the employee was taken to the doctor for evaluation and diagnosed with a shoulder sprain and placed on light duty.

Employee was working in the Gravel Bed Filter System, cleaning screens. He suffered a chemical irritation where his shirt sleeve rubbed against his wrist. Cement clinker dust, has a high pH and can cause an alkali burn. He was seen by the company doctor. Rx ointment was administrated. No lost time or restricted work (other than keep wound clean).

On 1/7/2008 Employee was unloading rail cars. As he was closing the rail car hopper door with a spud bar, he lost his footing and twisted his knee. He was taken to the doctor to be examined the following day, placed on restricted duty and given a prescription for pain.

Ee is alergic to bee stings. Ee self administered an epipen injection and was transported to Hosptital where he had a full recovery before the doctor could see him. No doctor report filled. There is some question as to whether or not this should be reported. We will report it as an "other" even though it was not medically treated.

Employee had complained about a lump in the palm of his right hand on 9/28 at 2:00. There was no problem at the time and he requested to see Dr. Joy who had seen him before. The doctor's next available date was October 6th. On Oct. 6 the doctor placed him on prescription drugs and refered him to a specialist. This is now a reportable accident as of October 6.

Employee had reported to work on September 6 stating that he had been seen by his personal physician and had been diagnosed with carpal tunnel syndrom. He had been working long hours, for serveral days, on an emergency overhaul and had aggrevated his wrist. He was sent to the company doctor who has placed him on light duty with PT and anti-inflamitory drugs.

Employee was checking the oil on the loader when he slipped. He was able to catch himself but wrenched his thumb. He reported the accident and was given first aid. On 10/2 he was seen by a doctor. The doctor released him back to his usual and customary. On 10/26 it was discovered that he had been given Rx drugs. This is reportable.

Employee was standing on concrete slab covered with broken rock and dirt. Turned and fell to the ground. Wrenched shoulder (dislocated) and bruised back. Was placed on light duty for about 4 weeks.

Employee was cutting bolts from a liner with an oxyacetylene torch when his pant leg caught fire. He sustained second degree burns to his shin in an area approximately 2 by 5 inches. He was wearing polyester coveralls which contributed to the severity of the burn.

Employee has been diagnosed with "trigger thumb? this condition could be the result of him using his right thumb to press the space bar. He performs clerical work. He received cortisone injections (medical treatment beyond first aid) but has not been restricted from doing his usual and customary work. This case is reporable because of the injections.

Employee was manually cycling tipping gate when cam drive pinched his finger against the actuating arm. He had removed guarding and had not stopped cam drive motor. He worked on tipping gates while gates were in operation.

EE was climbing into this 50 ton haul truck from the ground. He had walked through some mud and had buildup on his feet. As he climbed the ladder his feet slipped and he fell while holding onto the ladder. He bruised his arm and strained his shoulder. He was seen by Doctor's on Duty and placed on light duty.

Employee strained his groin on May 11, did not report to his supervisor until May 19, requested to see doctor on May 22, Ee was installing a traction wheel on a bucket elevator in the finish mill. He was carring a piece of plate when he stepped onto loose clinker. He says he felt a twinge, but did not report it at the time.

Ee was cleaning build up from the inside of a distribution manafold on the slurry line spray nozzle. She used compressed air to blow out the line and the plugged line blew material back into her face and into both eyes. This required medical care to remove the alkali material.

Employee was injecting compressed air into a discharge pipe beneath the drop out box when hot dust "spashed" into the opening around the gauntlet on his heat reflective gloves. This resulted in a second degree burn on his right hand. He was placed on light duty, to keep hand clean and dry.

Ee complained of sore arm about a week after the event (4/10/06). On 4/12 he went to see his own physician who told him that he had a strain and that it was work related. On 4/13 he was sent to our dr. who diagnosed a strain. PT and light duty prescribed.

Employee was placing a belt winder on B Belt. B-Belt is on an incline. The winder slid down hill pinching the Ee's hand between the winder and the open end of a 2" pipe handrailing. The railing had been removed to allow the winder to be placed into position. The resulting injury (an avulsion) required medical care (sutures)

Employee backed a D10 dozer over the berm on Bench 8 and fell to Bench 9, 35 feet below. He suffered minor lacerations on his face and a strained shoulder. He was placed on light duty.

Ee worked a double shift starting at 3Pm on 3/7 thru 6AM 3/8. By the end of the shift he was stooped over and walking strange. He reported it to the shift forman and told him he was "stiff from the cold night air". He went home and could not rest. Called the plant about noon and requested to be seen by a doctor. No specific incident remarks. Ee placed on light duty

DRIVING PICKUP TRUCK TOO FAST FOR ROAD CONDITIONS. HE HIT A RUT WHICH CAUSED HIM TO LOSE CONTROL AND DRIVE UP OVER THE BERM. HE JUMPED OUT THE WINDOW BEFORE THE TRUCK ROLLED OVER THE EDGE AND DOWN 40' SLOPE TO PIT FLOOR. 4/7/06: NO LOST TIME, NO RESTRICTED DUTIES, NO MEDICAL TREATMENT.
