Employee had worked an entire day operating a John Deere 750C Dozer along side another experienced dozer operator. At approximately 5:25pm the employee was last seen crossing over a dirt pile. A few minutes later that same employee saw the 750C dozer blade first in a ditch with the tracks still running and employee lying face up behind the machine.

EE WAS USING A WENCH TO OPEN DOORS ON THE BOWL MILL, THE SAFETY LATCH DID NOT FULLY ENGAGE AND THE WENCH HANDLE BACK SPUN AND STRUCK THE EMPLOYEE'S RIGHT INDEX FINGER CAUSING A FRACTURE
EE was checking Off Spec Door on # 4 Grizzly on the # 4 Kiln. The door was stuck in the open position and the EE pulled on the door to get it back on the hinges. He felt a pull in his lower back. Conditions and work conditions were normal, no Slip, Trip or Fall Hazards and Poor Housekeeping was NOT noted.
EE was leaving his work location in the Shipping Tower, decending the steps from the 3rd level. Prior to reaching the 2nd level landing, he looked back up at the 3rd level and noticed he had left the Load Out Chut in the down position. He was not paying attention to the steps and and missed the bottom step, twisting his right ankle, examined by MD, put on restricitons.
WHILE INSTALLING SHOTCRETE IN ROTARY KILN, EMPLOYEE WAS STRAINING TO GET A CLAMP ON PUMPING HOSE. FELT PULL IN CHEST. CONTINUED TO WORK FOR A BIT THEN LEFT JOBSITE. COULD NOT BREATHE, HAD CHEST PAIN. EMPLOYEE WAS TAKEN FOR MEDICAL ATTENTION.
Employee (Bowl Mill Attendent), was attending the clay feed belt, noticed large piece of clay, he stopped the belt and attempted to remove clay without assistance. He felt pain in his lower back, notified Supervisor. He was taken to local hospital - Diagnosed w/ back strain - light duty release to work 14 days.
The employee was preparing to grease a bearing on a conveyor. He bent over at the conveyor and strained his lower back.
Employee was climbing up on a dozer to enter the cab when his foot slipped off the first rail and went between rail and track causing sprain to right ankle.
Employee (mech) installing cylinder on scraper. After getting out from under it, felt a catch in his back. Occupational Doctor on 7/25/06 ? diagnosed w/ minor lower left back strain. Also given 10lb lifting restriction - later clarified on 8/11 to be because of back strain not an unrelated personal hlth issue.
The employee was tightening bolts on a crusher head. While he was pulling on the torque wrench, his footing and wrench slipped causing a lower back strain.
Employee was unjamming a lightweight aggregate log in #3 cooler that was formed by an upset in the kiln. He was using an iron bar (10' long, 10lbs) to pry the log in the cooler. He was standing outside the cooler on a platform & when the log loosened/broke free, his momentum carried his left hand down onto the handrail resulting in left finger tip fracture & dissolvable stitches.
The employee was removing a lump of clay from a conveyor. The weight of the clay was approx. 15 lbs. As the employee reached to remove the clay from the belt he felt a pull in his right arm (bicep). The employee has a possible torn tendon in the right arm next to the shoulder.
The employee was preparing to trim skirts on a belt conveyor. The employee exposed the blade of the cutter before positioning himself in firm working position. As the employee tried to position himself to complete the work he punctured and lacerated the left wrist.
The employee was preparing to replace a door on the back portion 6X16 Shaker Screen. The employee pulled a muscle on the right side of his back.
The employee was preparing to climb a ladder and lost his footing on the bottom step. The employee slipped and fell about 8" onto his bottom.
The weather before the start of the employee shift was humid and there had been previous heavy rains. The employee was decending on a flight of stairs. The employee slipped causing the employee to fall down the steps. The employee had bruising of the rt. hip and rt. forearm.
On 9/21/04 empl. reports back pain attributed to a work accident on 3/9/03. That day lifted 10lb can, strained back. 3/10/03 and 9/27/04 Dr visits status: perform normal job. Visits on 10/6 & 10/8/04 Dr noted no change, refer to specialist exam 10/12 & MRI 16th. MRI & diagnosis was Lumbar Spondylosis. Empl. could contin. to work w/limits on lifting, bending, etc.
EQUIPMENT REPAIRS, EMPLOYEE HAD TWO FINGERS CUT.
The employee was preparing to change a hydraulic line on a WA 450 loader. The line was in a area that had limited access. The employee turned to access the ladder and felt a pull in his lower back.
EMPLOYEE WAS ASSISTING WITH THE SET UP OF A CRANE FOR DRAGLINE. WHILE STANDING ON THE TRACK HELPING TO GUIDE THE MAIN HOIST INTO POSITION IN ORDER TO PUT GANTRY IN PLACE, CABLES THAT WERE ATTACHED TO THE GANTRY WERE PULLED CAUSING THE GANTRY TO TIP & FALL. PART OF THE GANTRY HIT HIS LEG RESULTING IN A GROSS CONTUSION & SPRAINED RT. FOOT.
After loading the clay bin, EE entered maintenance shop. He placed gloves on his hands to begin check of 938 Caterpillar. Moments later, he felt the glove on his right hand tightening. EE removed the glove on his right hand and noticed swelling in a centralized spot on his wrist. He also noticed a black dot in the center of swelled knot on his wrist.
We had a coal bin fire and were in the process of dumping the coal into loader buckets for disposal. The injured party was opening a dump valve when a small piece of floor grating shifted and allowed the employee to fall through to the ground below. The employee sustained a laceration to his head approximately 2 inches long requiring 10 stitches.
THE EE WAS CUTTING SKIRTS FOR A BELT CONVEYOR. THE BOX CUTTER SLIPPED FROM THE SURFACE OF THE BELT, AND CUT THE EE'S LEFT LEG.
THE EMPLOYEE WAS CLIMBING UP THE LADDER ON A FRONT END LOADER WHEN HE FELT SOMETHING POP IN HIS LEFT ELBOW. THERE WAS NO TRAUMA TO THE ELBOW. NO DEFECTS WERE FOUND BUT THE INVESTIGATION IS CON TINUING.
IMPROPER BODY POSTURE. THE EE WAS REACHING FOR A ROLLER HOUSING AND PULLED A MUSCLE IN THE UPPER RIGHT PORTION OF HIS BACK.
AN EE OF AN OUTSIDE VENDOR WAS SECURING AN OLD MOTOR GRADER TO A LOW BOY TRAILER FOR TRANSPORT OFF SITE. AN EE OF ANOTHER CONTRACTOR RAN INTO THE AFOREMENTIONED EE WITH A 400 SERIES CATERPIL LAR TRUCK. BOTH INVOLVED PEOPLE NOT EMPLOYED BY BIG-RIVER INDUSTRIES.
THE EE WAS POKING AT MATERIAL THAT WAS IN AIRGATE. WHEN THE EE CONTINUED POKING HE PULLED SOME MUSCLES IN HIS SHOULDER.
THE EE WAS RUNNING UP THE STAIRS OF THE CRUSHING DEPT. AS THE EE WAS RUNNING HE TWISTED HIS ANKLE.
EE WAS STANDING ON THE TRACKS OF A BULLDOZER AND SLIPPED. HE FELL TO THE GROUND FROM THE TRACKS.
THE EE THREW THE OFF SPEC CHUTE. WHEN THE KILN GOT BACK ON SPEC, THE EE HAD TO PULL THE CHUTE BACK UP IN ORDER FOR THE MATERIAL TO GO TO THE STOCKPILE. WHEN THE EE THREW THE CHUTE, HE PULLED A MUSCLE IN THE MIDDLE OF HIS BACK.
EE WORKED NORMAL SHIFT AS A CLAY FEED ATTENDANT & WENT HOME. HE NOTICED SWELLING AROUND HIS RIGHT ELBOW. THE DOCTOR SAID HIS SWELLING IS FROM BURSITIS FROM AN OLD INJURY 4/25/00.
THE EE WAS CLEANING & SHOVELING UNDER THE TAIL PULLEY. WHEN THE EE STOOD UP FROM SHOVELING HE FELT A PAIN IN HIS UPPER & LOWER BACK.
THE EE WAS SPLICING A WIRE ON TOP OF THE #3 KILN BACKEND. THE WIND STARTED BLOWING DUST FROM THE STOCKPILE & THE KILN BACKEND. NO MATTER WHAT DIRECTION THE EE TURNED, THE DUST STILL GOT IN HIS EYES.
THE EE WAS CLIMBING DOWN OFF OF THE TRACKHOE. WHEN THE EE PUT HIS FOOT DOWN HE THOUGHT HE WAS STEPPING DOWN ON SOLID GROUND. BUT THE GROUND WAS EXTREMELY SOFT. WHEN THE EE STEPPED DOWN INT HE MUD, HIS FOOT SANKED IN THE MUD AND HIS FOOT TURNED TO THE SIDE. THIS CAUSED HIM TO BE OFF BALANCE AND TO PUT TOTAL WEIGHT ON ANKLE.
EE WAS WALKING DOWN CATWALK WHEN HE FELT SOMETHING CATCH IN HIS RIGHT KNEE, TAKEN TO CLINIC STRAINED KNEE.
THE EE WAS CLIMBING DOWN OFF THE CLAYSHED LADDERAFTER A HOSE HAD COME LOOSE.
EMPLOYEE WAS REMOVING A LOG FROM #4COOLER WHEN PRY BAR SLIPPED AND JAMMED FINGER AGAINST HAND- RAIL. RESULTING IN LACERATION TO FINGER. STITCH-ES REQUIRED.
THE EMPLOYEE WAS CLEANING UNDER THE CRUSHER WITH THE BOBCAT. THE EMPLOYEE WENT NEAR THE EDGE OF THE GROUND TO CLEAN AND CROSSED SOME UNLEVEL GROUND. WHEN THE EMPLOYEE CROSSED THE UNLEVEL GROUN D HE FELT HIMSELF ABOUT TO TIP OVER. IN REACTIONTHE EMPLOYEE STUCK HIS HAND OUT OF THE BOBCAT AND GOT IT SMASHED BETWEEN THE BAR AND NEARBY CAT WALK.
EE JUMPED FROM THE LEDGE AROUND THE #3 COOLER AND CAUSED PAIN TO HIS RIGHT KNEE.
EE WAS SHOVELING UNDER THE #2 TAIL PULLEY & THE EE BENT UNDER THE CATWALK TO GET TO THE OTHER SIDE. WHEN THE EE TRIED TO GET UP HE FELT A POP IN HIS BAKC
EMPLOYEE WAS SUPERVISING A PROJECT WITH FITTING AND WELDING TAKING PLACE. EMPLOYEE RECEIVED FLASH BURN TO BOTH EYES.
THE EMPLOYEE WAS TRYING TO WARN ANOTHER EMPLOYEETHAT WAS ON A FORKLIFT WITH A MOTOR. THE FORKLIFT GOT HUNG ON THE DOOR. THE EMPLOYEE NOTICED THAT THE BOOM AND THE MOTOR WERE HUNG ON THE DOOR AND TRIED TO WARN THE OPERATOR. IN THE PROCESS THE EMPLOYEE'S HAND WAS WAVING BETWEEN THE STUCKMOTOR/BOOM AND THE I-BEAM. THE BOOM CAME LOOSE AND THE EMPLOYEE WAS HIT IN THE HAND.
EMPLOYEE WAS RUSHING DOWN STEPS AND TRIPPED LANDING ON HIS KNEES. THE MEPLOYEE BRUISED HIS LEFT KNEE.
THE EE WAS FEEDING THE HOPPER AND NOTICED THAT THE FEED WAS NOT GOING UP THE BELT. THE EE GOT OFF THE LOADER AND RAN TO THE HOPPER TO CUT OFF ALL THE CONTROLS. IN THE PROCESS OF THE EE RUNING TO HOPPER. HE TWISTED HIS RIGHT ANKLE.
THE EMPLOYEE WAS TRYING TO UNSTOP A CLAMGATE. THE EE TRIED TO UNSTOP THE CLAMGATE BY USING THE HANDLE. THE EE'S HAND SLIPPED OFF THE HANDLE. THIS CAUSED THE HANDLE TO COME BACK UP AND HIT T HE EMPLOYEE IN THE HAND.
EE WAS PULLING A SECOND BATTERY FROM A FORKLIFT.THE WEIGHT OF THE BATTERY WAS PLACED ON HIS RIGHT ARM. THIS CAUSED HIM TO FEEL A PULL & BURNING SENSATION.
EE ALLEGES THAT THE SEAT ON THE 613C CATERPILLERSCRAPER CAUSED AN INJURY TO HIS LOWER BACK
EE WAS PUSHING DIRT CLODS ON GRILL BELT WHEN A LARGE PIECE OF DIRT CAME ON BELT. WHEN EE WENT TO PUSH DIRT OFF BELT, HE PULLED A MUSCLE IN HIS BACK.
