AFTER TIGHTENING BEARING LOCKING SCREWS ON CONVEYOR, EMPLOYEE TAPPED THE BEARING COLLAR WITH A HAMMER TO SETTLE IN THE SCREWS FOR RE-TIGHTENING. WHEN HE TAPPED THE BEARING A PIECE OF THE COLLA R CHIPPED OFF AND LODGED ITSELF IN HIS RIGHT FOREARM.
