Showing all 22Employee was exiting a tool trailer onto the top of a 4ft ladder. EE lost EE's balance, jumped down and twisted EE's knee. EE sought medical treatment and on 7/11/25 EE was Dx with a ACL tear.
Employee was working near screen plant. Wind blew sand underneath safety glasses. On 05/07/2025 we were notified the Employee sought treatment and was Dx with a corneal abrasion. Employee was prescribed eye drops. Reporting within 10 days.
Employee was moving material away from the cone crusher. Cone crusher was shut down to perform this task. A piece of 6" aggregate fell on the tip of right index finger. Employee sought treatment and was diagnosed with a fractured finger tip.
EE swung the hitch down on the conveyor to clean any debris out of it as EE was preparing it for transport. EE swung the hitch back up into place & hit left hand. EE sought medical treatment & was diagnosed with a bruise. EE was taken off work.
Employee was installing flashing and slipped on the work surface, causing weight to shift awkwardly. Employee sought medical treatment and was diagnosed with a hernia. Employee continues to work. Surgery has been set for 11-2-22.
Rock stuck in conveyor belt. Employee locked out tagged out machine. Employee then used pry bar to remove rock and pry bar slipped - cutting left thumb. Employee sought treatment and received stitches and antibiotics.
Another employee was moving a chute with a skid steer. The chute was chained & the skid steer hit a bump which caused the chute to shift in the rigging. The chute struck the employee's left foot. EE sought medical treatment & was diagnosed with 2 broken toes. EE was wearing steel toes.
Employee had annual audiogram performed on 12-12-18 and results showed possible threshold shift. Employee was sent in to be retested on 1-21-19 to verify the shift. After the audiologist reviewed the most recent test results we received notice on 1-29-19 confirming the threshold shift. Employee is not seeking medical treatment at this time.
Employee was walking around the plant & while doing so a rock bounced off a conveyor at a transfer point & hit left hand. They sought treatment & was diagnosed with a bruise & given a splint.
Employee was stung by wasp while preparing for plant move. While employee was being treated at Urgent Care employee lost consciousness. Employee was released a few hours later after being treated at the hospital.
An employee was getting into a skid steer on 5-28-14 and slipped causing him to fall and land on the skid steer floor. He did not seek medical treatment until 12-6-14 when he saw a chiropractor.

Employee was drilling. He opened door of driller & a can of paint fell out. He got up to pick up paint - bent down & when he stood up, he hit his forehead/temple on the door of driller. He was wearing hard hat. He was unconscious.
Employee was welding with protective gear on. Later that night his eyes were bothering him. Doctor said he had flash burn.
CLEANING CHUTE ON BINS. LOST FOOTING AND JAMMED KNEE. THE RESULT WAS TORN MENISCUS. SURGERY.
CHECKING BELT UNDER THE ALLIS DUAL SCREEN. WAS UNDER LEFT SIDE OF ALLIS LOOKING UP AT ALLIS DIRT HAWG AND CHECKING CLIPS, WHEN COMING BACK OUT, A ROCK CAME OUT FROM TOP OF SCREEN AND HIT THORA CIC MUSCLE ON LEFT SIDE OF BACK. I FELL TO GROUND AND NOTICED SOFTBALL SIZED ROCK TO THE LEFT OF ME.
