While on ground level, the EE reached into an excavator for a hoodie, lost footing, and grabbed the handrail, injuring their left elbow. Sought treatment the same day and returned to work with restrictions that did not affect regular job duties. An 08/15/25 MRI showed a left triceps rupture requiring surgery. No equipment, compliance, training, or PPE issues were involved.

The operator was using and excavator to lift a load and the material slipped on the employees hand when employee tried to put lifting strap under load.

While attempting to clear a buildup of material in a chute, the EE used a pry bar to dislodge the obstruction. During the process, the employee struck downward with the bar, causing the material to break loose unexpectedly. As a result, the EE's hand made contact with the side of the chute, leading to a laceration on the middle finger of the right hand that required three sutures.

EE was changing bottom rollers on the Komatsu D65 Dozer when a roller fell on EE's right index finger. Resulting in a fractured right index finger.

Employee was attempting to unplug a dust bin chute when the material shifted causing pain in EE's lower right back. The incident did not involve any equipment or compliance issues. It also did not involve job skill, proficiency, training, PPE or protective devices.

CDL delivery driver was complaining of chest pain while off site making a delivery. EMS was called when EE returned to site. EMS transported EE to the hospital where EE was diagnosed with a non-life-threatening illness. The illness was determined to not be work related.

Employee arrived onsite and parked in the employee parking lot to start EE's workday. EE was experiencing what was described as chest pain and shortness of breath. EMS was called and the employee was transported to the local hospital for tests and observation. Note: the employee did not exit EE's personal vehicle and did not clock in for EE's shift.

While attempting to reposition the flange holes of a 12" HDPE pump line, the EE removed the last bolt and the pump line shifted and hit EE in the lower left leg. The incident did not involve any equipment or compliance issues. It also did not involve job skill, proficiency, training, PPE or protective devices.

Employee was helping install a steel plate in a feeder when the plate got wedged. The employee then tried to jerk it free and felt pain in thoer back. Employee went to the doctor to be seen for acute low back pain.

Employee was welding when a piece of slag burned through employee's pant leg and down into right boot, burning the outside of right ankle. Employee continued to work until 7/23/2019 when employee was taken away from work because of the burn for three days and returned to normal duty on 7/26/2019.

Employee broke right pinky finger while changing screens.

Employee was off duty and lives with wife in camper on mine property behind shop. Per wife who was with employee at the time, employee had just taken shower and was laying down. Wife heard noise. Found husband unresponsive. Summoned help from employee at the shop. 911 was called and transported him to Augusta County Regional Medical Center.

Employee reported that while performing maintenance on speed reducer on conveyor belt C-6, he felt pain in his right hip and back.

Employee reported that metal plate he had just cut, fell on his foot just above the steel toe.

Employee reported he stained his back while replacing anvils in CEMCO Model 80 crusher.

Employee reported that he was driving rebar into the ground with a sledge hammer when his swing missed causing the top of his right hand to come in contact with second piece of rebar. Injury resulted in 13 stitches.

Employee reported that he was tightening come-along while performing maintenance on #2 crusher when anchor chain failed causing him to fall and cut the back of his head. Injury required 5 staples.

Per coroner, miner died of natural causes.

Employee was in vacinity of welding operations. Was notified prior to welder striking electode. Employee closed eyes and shielded eyes with hardhat. Employee completed shift and began having eye pain at home. Employee went to Emergency Room later that evening where he was diagnosed with Ultraviolet Keratitis (welder's burns).

Employee was shoveling under jaw crusher. Employee was bending over and twisting when he felt pain in lower back.

Employee was dismounting haul truck when his wedding band caught on the edge of the top step, causing ring to cut into his finger.

Employee was welding on excavator bucket supported by three pieces of wooden cribbing. Bucket was attached to excavator. Two of the three pieces of cribbing split causing the bucket to suddenly drop. Employee was struck in the knee by a piece of cribbing.

Employee was attempting to pry a metal beam into position, the beam dropped down catching his left thumb.

Employee was attempting to loosen compacted material in a storage bin by forceibly banging the metal release handle against the bin. While doing so he caught (pinched) his finger between the handle and the metal crossbar associated with the bin.

Employee was changing air filter on Komatsu Portable Jaw Crusher, when he attempted to get up from a kneeling position, at which time he was unable to do so.

Was working on Ex-tec turbo mahcine and was in an odd position under screen, tightening bolts on screen.

Employee was removing scraper assembly from jaw crusher when the U bolt broke causing scraper to fall on left hand.

Using a crowbar to turn the tailpulley over on the #2 belt, the crowbar broke loose smashing his right hand against the angle line.

Employee was running a rock crusher, when a rock hit him in the back of head under his hard hat.

Employee said he was shoveling a rock at the plant and pulled his chest muscle.

EMPLOYEE WAS WORKING ON #3 SCREEN STARTER BOX WHEN AN ELECTRIC COIL FLASH OCCURRED.

BENT OVER TO PICK UP A ROCK WHEN LEFT SIDE OF BACK BEGAN HURTING.

EE WAS GETTING INTO SKID STEER LOADER WHEN HIS PANT LEG GOT CAUGHT ON THE LEVEL CAUSING THE SKID LOADER TO GO INTO MOTION THROWING HIM INTO THE CAB OF LOADER *PARKING BRAKE WAS PULLED

EE WAS UNDER WATER TRUCK PUTTING A MUFFLER ON WHEN DIRT FELL DOWN INTO HIS LEFT EYE

BACK WAS HURT WHILE REMOVING CHUTE FROM SCREEN AT LIME PLANT WITH ANOTHER EE.

EE WAS HELPING ANOTHER EE PUT CEMENT BLOCKS ON THE GROUND WHEN HIS LEFT HAND GOT CAUGHT BETWEEN 2 BLOCKS.

EE WAS PICKING MUD OF BELT WHEN A BIG CHUNK CAMEDOWN BELT. HE TRIED TO FLIP THE CHUNK OF THE BELT. THE CHUNK OF MUD CAME DOWN ON HIS LEFT HAND AND BENDING HIS MIDDLE FINGER SIDEWAYS. HE HEARD & FELT A POP SOUND.

EMPLOYEE WAS IN HIS LOADER, LOADING ROCKS ON TRUCKS WHEN HIS CHEST STARTED HURTING. HE BEGAN SWEATING PROFUSELY, BREATHING RAPIDLY. HE SLUMPED OVER STEERING WHEEL. WHEN WITNESS SAW HIM HE WAS HOLDING HIS CHEST WITH HIS RIGHT HAND, LEFT ARM HANGING DOWN SHAKING, HEAD SHAKING, TEARS COMING OUT OF HIS EYES.

WAS STOCKING STONE IN QUARRY WHEN TAILGATE DID NOT LATCH PROPERLY. HE STOPPED THE TRUCK & WENT TO LATCH TAILGATE WHEN THE LEVER FELL UP HITTING HIM ON LEFT ELBOW. ICE WAS APPLIED TO ELBOW. WEN T TO STAUNTON CLINIC TO SEE DR WOODWARD ON 6/19/02. ARM WAS PUT IN SLING - BADLY BRUISED - RESTRICTION.

WAS PUTTING A TOOTH IN THE JAW CRUSHER WHEN A ROCK BROKE LOOSE INSIDE PULLING EE'S LEFT ARM DOWN INTO CRUSHER. STEEL WIRE BROKE LOOSE CATCHING HIM ON LEFT SIDE JAW AREA.

RE-INJURY TO AN OLD INJURY. HAD OPERATION MAY 10, 2002.

EE WAS HITTING DRILL STEEL WITH SLEDGE HAMMER. HE MISSED THE DRILL STEEL & HIS KNUCKLE WAS POSITIONED BETWEEN DRILL STEEL & HAMMER.

EE WAS HELPING A TRUCK DRIVER OFFLOAD EXCESS FERTILIZER FILLER & TAILGATE CLOSED ON HIS RIGHT HAND

EMPLOYEE WAS WORKING IN LAB; KNOCKED A FLASK OFF COUNTER. AS HE TRIED TO CATCH IT, IT BROKE, CUTTING HIS MIDDLE FINGER ON LEFT HAND.
