Teammate while holding the Manual override with their right hand inserted their left index finger into the Borax Valve to see if it was clean. The teammates stated their right hand slipped off the controls and the valve closed and amputated approx 1" of EE's left index finger.

The teammate was attempting to dump the hopper. EE was unable to access the containers on the paved, North Side of the lot. As EE tried to reverse the forklift to align with the hopper, the back tire came off the pavement and became stuck in the mud. EE tried to maneuver the fork truck out of the mud with the load lifted and the forklift turned over.

EE was attempting to get into the 2017 S570 Bobcat. While attempting to enter the Bobcat EE stepped in the mouth of the bucket and grabbed a hold of the handles to pull self into the Bobcat. While stepping up to the grates to enter the Bobcat EE's left foot slipped on the mud which EE was standing on in the Bobcat's bucket.

While lacing the belt and using the belt stretcher on line four EE was holding onto the frame of the belt stretcher with right and tightening the belt using left hand and it appears there was a small notch underneath the stretcher which EE was not aware of and when pressure was applied hand slipped into the notch on the Belt Stretcher which caused the injury..

Maintenance Technician stepped to the side onto an impact wrench that employee had laid on the ground while working on changing screens. Employee's ankle rolled when employee stepped on the tool.

While repairing the safety Latch that holds trailers in place at the dock bay the shock came off unexpectedly causing it to hit EE's right index finger and thumb, resulting in a cut that needed stitches.

Employee informed EE's Supervisor that EE would be moving slowly while working. The Supervisor asked why EE would be moving slowly and EE told EE's Supervisor EE bent over to lift a box from the floor and felt a pop in EE's lower back and that EE's right leg went numb. When asked if EE needed medical attention the employee stated no, EE would be fine.

While the Operator was talking he lowered his hands and the belt pulled his hand in between the free roller and the conveyor. As a result, the top of his left hand was scraped.

While trying to remove bags from the new long on the packaging line, the operator raised the guard and pulled on the bags and his finger was cut on the gear of the speed dial.

While repositioning tray on packaging line the operator reached beyond the guard and the sealer came down and pinched her finger between the tray and the jig. The Operator was released with work restrictions and is working. No lost time, but working restricted work days since 12/16/13.

The EE was working on a case packer unit. Cardboard boxes had become jammed together. The EE locked out the machine and released the stored energy, reached in with hands to take out jammed boxes while pulling on a box his left hand slipped off of the box flap and struck a jug guide flap. This caused a laceration to middle finger on left hand.

Cutting thin pipe with a 4.5 grinder cutting wheel, repositioning grinder guard did not let grinder come to a complete stop and unplug. Caught glove between grinder and cutting wheel and pulled finger onto wheel cutting his finger.

The employee swept up a small pile of trash. He left the area and took a break. When he returned to the area, he stepped on part of the trash pile. His right foot slipped on a plastic jug cap (in the pile). Due to the twisting motion, it caused an ankle sprain.

EE was operating an automatic Bemis stacking machine. A filled pallet of cat box filler was exiting the stacker on an exit conveyor. The pallet had a broken board on the bottom. The pallet hung on the conveyor. EE used a pry bar (metal pipe-1 1/4" pipe 5 1/2') to try to unhang the pallet. As he pulled on the pipe, he felt a sharp pain in his lower back & right leg & hip.

The employee was placing a roll of stretch film on a stretch wrap machine. The stretch wrapper arm was not in the correct loading position and he reached out while holding the roll of film (38.4) lbs). He said that he twisted his upper body while placing the film. EE heard a noise coming from an overhead conveyor. He walked over to the head pully of the conveyor & looked up. A small fragment of metal fell from the head pulley of the conveyor and struck the EE in the right eye. The head pulley of the conveyor had slid over against the frame causing a shearing action of metal.
