ROD IN CRUSHER BROKE AND FLEW INTO THE MECHANICS FACE AS HE WAS REPAIRING THE EQUIPMENT. THE ROD BROKE BECAUSE HE WAS HAMMERING IT TO ADJUST IT. THE METAL ROD STRUCK HIM IN THE RIGHT SIDE OF H IS FACE CAUSING BRUISING AND LACERATION.

I,NAME WAS STAN8ING IN THE BUCKET OF THE LOADER TO FIX THE BOLTS ON THE HOPPER OF THE CRUSHER. LOADER ROLLED BACKWARDS, I,NAME SLIPPED ON LEFT FOOT TRIED TO CATCH BALANCE WITH RIGHT FOOT LOSE BALANCE AND FE5L TO THE GROUND SIX FEET BELOW.
