Employee was washing the Caterpillar 226 skid steer loader. When he opened the engine cover the radiator cap came loose and hot water was spilled from the engine to his abdominal area.
After working hours employee stayed at company parking lot to perform oil change in his personal car. He was using a hydraulic jack and wood pieces block equipment from movement. The hydraulic jack failed and employee was pinned in the chest between car and floor. The security guard helped him lift the car back and employee got up normally.
Employee was tightening a bolt on a radiator panel of a CAT 988 Loader. The panel was standing and fell on top of the assistant mechanic.
The injured person was on top of his trailer placing the tarp on the load and attached to a safety harness, he noticed that hook started moving inadvertantly, he pulled and trying to step down the trailer he fell to the ground.

EE WAS USING A HAND TOOL TO MAINTAIN THE SERVICETRUCK. WHILE HE WAS COMING DOWN FROM SERVICE TRUCK, HE SPRAINED HIS RIGHT ANKLE.
WHILE THE EE, A WELDER, WAS WALKING TO THE SCREW WASHER AREA OF THE PLANT, HE TRIPPED AND FELL LOSING HIS HARD HAT AT THE MOMENT OF THE FALL, WHEN HE GOT UP HE STRUCK HIS HEAD WITH A STEEL CHA NNEL. THE EE SUFFERED HEAD TRAUMA WITH OPEN WOUND AND RIGHT ARM LACERATION.
ALLEGEDLY WHILE EE WAS CLEANING A WORKING PLATFORM WITH A WATER HOSE, HE STARTED WALKING DOWN A STAIR, WHEN HE SLIPPED & FELL IN THE LAST 5 STEPS OF THE STAIRS. ALLEGEDLY HE RECEIVED INJURIES IN THE BACK & FRONT RT.SIDE (RIBS).
EMPLOYEE IS A HEAVY EQUIPMENT OPERATOR, WHILE OPERATING CAT D11 STARTED TO FEEL PAIN IN HIS RIGHT WRIST. FURTHER WAS DIAGNOSED WITH SEAPHOID FRACTURE.
WHEN REMOVING A BOX OF BALLS USING A HAMMER, ONE PIECE OF STEEL BECAME EMBEDDED IN RIGHT KNEE.
THE EE USED A LADDER TO GET DOWN FROM THE MACHINE ON WHICH HE WAS WORKING. HE SLIPPED & FELL TO THE FLOOR, BRUISING HIS RIGHT ARM.
EE WAS PUSHING A PART AND HE HIT HIMSELF WITH THE CORNER OF THE MOTOR. EL EMPLEADO ESTABA EMPAJANDO UN PIEZA U SE GOLPEO CON UNA ESQUINA DEL MOTOR.
THE EE RECEIVED A SMALL INJURY WITH A PIECE OF METAL ON THE RIGHT FOREARM WHILE HE WAS MOVING THEM FROM THE IMARI LOADSTONE, THE MAGNET THE INJURY WAS RIGHT ABOVE THE AREA WHERE THE BLOVE DOES NOT COVER.
EE WAS GETTING DOWN FROM MACHINE. HE STEPPED ON TOP OF STONE & IT ROLLED. LOST BALANCE AND FELL.
EMPLOYEE WAS REMOVING PART OF THE TRANSMISSION FROM A MACK TRUCK TO REPAIR. HE FELL AND HURT HIS LEFT SHOULDER.
EE SLIPPED AND FELL HITTING HIS SHOULDER.
EE RECEIVED A BLOW IN THE RIGHT ELBOW.
