Employee was holding bolt on a pin being driven out by another employee with sledge hammer. The hammer glanced off the bolt striking the employees left hand and ring finger.

Employee reported that EE felt a "pop" in EE's left knee while stepping down from the platform on the deck screen. Was treated as a first aid case and full duty pending MRI results and if Ortho recommends surgical repair.

Employee was squatting, working on a rail guard on the crusher when they felt a pop in knee. After the initial doctors office visit, EE was prescribed self care. The knee did not improve and doctor requested an MRI. The MRI revealed a torn meniscus that would require surgery to repair. Employee underwent surgery on 10/7 was returned to modified duty following surgery on 10/10.

EE started pump at remote quarry location, turned, homeless man (been there before) was right there. Man grabbed/slammed EE to ground, ran away. EE's wrist injured as EE tried to break fall. Dr diagnosed as left wrist pain with restrictions still allowing EE to do normal duties. Follow up x-ray was taken, revealed small fracture/bone chip in wrist. The police were called.

Pulled hydraulic oil hose until reel locked-made swinging motion to toss hose on top of excavator. Felt twinge in upper left arm. EE got on excavator to loosen fill cap with wrench. Cap was screwed tight-so EE pulled on the wrench with both hands-felt another twinge. EE used different wrench, was able to loosen fill cap. On 10/4, EE requested to be checked by doctor.

Walking up hill and employee's right knee gave out. Until MRI was completed on 6/27/19, this was just 1st Aid and not an MSHA reportable incident.

After 34 years of working as a shop mechanic, EE was having issues with hands. When ee went into family doctor, and subsequent testing it was determined that ee has developed carpel tunnel syndrome. It has now been determined since it was filed with work comp on 5/8/17 that at least partly through time working as a shop mechanic is part of the cause.

Employee has had back issues previously, started hurting again & having leg pain while working. He requested to go to the dr on 7/23/14. He continued to work his full work duties while continuing to see dr and going through physical therapy. The dr decided to do surgery 10/7/14 and no work at least until 11/10/14. When employee can rtd this form will be updated

While fueling a loader, staged away from the quarry, fueler slipped on ladder attached to loader, falling to ground, injuring elbow. Weather conditions were wet/rainy and fueler's gloves were wet, causing the fueler to lose his grip on the ladder.

Original date he felt his knee pop when he kneeled. Had it looked at by drs and MRI. Dr found nothing but told patient that he could do exploratory surgery if it did not get better. This was not a reportable accident until 7/9/14, when surgery was done. Dr removed scar tissue and a pillia--outpatient surgery and able to report back to work his next working day.

Stepped out of truck, twisted right knee. Was originally diagnosed as a sprain. Continued working. 4/19/13 Dr requested MRI. On 4/24/13 patient met with Dr about MRI and it was determined that he has a torn meniscus. On 4/25/13 patient is put on lost time and surgery is pending.

Was using sledgehammer, felt strain in his back. Has been seeing Occ Med, but no medical treatment was prescribed. Because of continuing random pain, and previous laminectomy from home injury, he requested MRI which was read 4/22/13. Assessment lumbar strain, will now receive steroid injections. He continues to work on his regular job, but now with some restrictions.

Employee first reported injury on 3/31/11. Stated he had used hammer on 3/21/11 and later in the day felt pain in right shoulder. Thought he would improve, but felt it was still sore. Saw dr on 4/1/11, dr stated it was a right shoulder strain, gave him an injection and put him on light duty.

Employee was in process of removing a steel hopper side. He was cutting what he thought was 1 of last 4 bolts securing panel when it was actually the last bolt. He may have been leaning on panel when it gave way creating opening causing him to lose balance & fall thru opening 21ft to ground. He was wearing fall protection but was not tied off to the provided life line.

EE reported hurt his right knee 10/30/09 approx 12:00PM. EE reports he was walking across rocky ground , rock broke & he fell backwards. EE said when he fell he twisted right knee, it hurt at the time but didn't think much of it. 11/9/09 went to dr for check, put him in knee sleeve. 11/16/09 dr gave cortisone shot. No lost time, is working.

Inside 1516 Andreas Rock crusher working on loose liner on curtain inside crusher. Used 3 lb hammer to beat on top of liner to loosen it up from material buildup. As liner loosened, hit it from bottom to knock it out and a piece of metal slivered off liner and went into right bicep.

Working in shop, hammering with left hand a U-joint into yoke, holding U-joint with right hand. Missed U-joint and hit web of right hand (between thumb and index finger) with hammer. Stitches were needed where hammer struck hand. This happened at the end of the day starting a four-day holiday weekend, there was NO lost-time.

Was cutting in between two decks on screen. Hot slag from cutting bounced onto side of screen and bounced into his ear. He did not report the accident until 12/26/07. He was sent to urgent care to be checked. Urgent care Dr made appt for him on 12/28/07 with an ENT. He was checked and given antibiotics. NO LOST TIME.

Welding rod stuck between 2 tires, couldn't reach it used foot to kick it.Burned thru rt boot.He kept area clean, used antiseptic.7/26 it became red & swollen went to Urgent Care.Diagnose w/quarter-size burn/puncture between big & 2nd toe w/cellulitis.Wound debrided, given antibiotic & Slivadene burn cream-told to clean twice/day.Allowed to return to work w/out restrictions.

EE has repetitive motion pain in his hands. We have been working with him all summer. He has completed two operations on both hands. He was off 7 days so far but will add more. Surgery on Sept 28th - this illness has been long and painful.

EE HAD BOTH SHOULDERS OPERATED ON IN 97. ON 7-2-01 HER RIGHT SHOULDER STARTED TO TIGHTEN UP WHEN SHE WAS STEERING HER TRUCK. WE MADE AN APPT WITHT HE DOC WHO RELEASED HER TO GO BACK TO WORK WI TH SOME REST AND MODIFIED DUTY. SHE HAS HAD MOREPROBLEMS WITH HER SHOULDER. SHE HAS BEEN TOT HE DOCTOR SEVERAL TIMES, THE SHOULDER IS NOT GETTING BETTER.

EE WAS PULLING ON THE ROPE FOR HIS TARP ON THE TRUCK #119. THE ROPE BROKE AND HE FELL BACKWARDS AND PUT HIS LEFT HAND OUT TO CONTROL FALL. HIS ARM ABSORBED ALL OF THE ENERGY. HIS LEFT ARM BROK E AT THE WRIST.
