Changing the drive drum on a stacker. A pre-assembled drum was rigged to a forklift. Trying to get the drum inside the stacker frame, they pushed on one side of the drum the rigging slipped on one end. The employee was trying to guide the drum on that side. The employee's finger was smashed between the shaft and the stacker frame. EE's left index finger suffered an open fracture.

Removing Bull Wheel Bolts on Jaw Crusher, employee swung sledge hammer to hit wrench that a second employee was holding. Hammer deflected off of the wheel and hit the second employee's left little finger.
MINER WAS ASSISTING FOREMAN TO REMOVE PIECE OF IRON FROM BEING STUCK IN THE CONE. MINER'S LITTLE FINGER ON RIGHT HAND WAS PINCHED BETWEEN PIECE OF IRON AND THE CONE. POSSIBLE FRACTURE.
