Operator was emptying a hydrated lime bin. When EE dumped the bin, the material covered EE's feet and up EE's legs. The operator turned to get out of the material and fell face first in a pile of hydrated lime. The hydrated lime mashed around and under EE's safety glasses going into EE's eyes.

Employee was walking on a grade when they slipped and fell on compacted snow. The employee became sore at the time of the fall. Employee went to the ER later that day on January 10th and was checked out and released. Employee went to the ER on January 26th at 12:46 a.m. and was then placed off of work.

While testing grounding systems an arc flash occurred at the disconnect causing a burn to the employees left hand and flash burn to the eyes.

Employee was cleaning the bottom of a chute for solid fuel. Employee was using a 6' long hand held jack hammer bit to break the material free. When the material came loose it drug the jack hammer bit down contacting a belt conveyor. The bit was jerked out of the employees hands and made contact with the side of EE's head, causing a small cut on the left middle finger.

Operator was trying to clean a blockage out of the bottom of a bag house compartment. A chunk of material fell onto the operators hand pushing it into the roll feeder cutting the ends of two fingers on EE's left hand.

Operator was using a T-Bar to dislodge material from a head chute when the material gave way the weight of the bar traveled down and the operators thumb was struck between the T-Bar and the head chute causing the operator to suffer a broken thumb.

Operator was climbing up dozer to enter the cab when EE lost EE's footing and slipped on the step causing EE to fall and hit EE's mouth on the handrail. Operator chipped EE's front tooth on the handrail. Steps were wet from the rain the previous night.

Miner stepped off railcar injuring right knee on 7/31/19. EE reported the injury to operator on 8/3/19. EE has remained on full duty, per doctor orders, until they had surgery on 10/1/19 at which time EE was taken off duty until deemed fit to return.

EE was cleaning dried (mostly) mud from around the pedals of the plant Bobcat loader prior to use...EE was using a trowel to dig the mud and chip away at the dried material...EE's hand slid down the handle and the tip of the right ring finger struck the back of the trowel...(4) stitches and released to return to work with no restrictions

Employee was training with the oiler moving barrels when EE mashed hand between two barrels. EE was taken to the doctor for diagnostics. X-rays were negative. EE had a contusion on left hand. EE was placed on light duty until 9/28/2017. Employee worked through 9/22/17 and then never returned.

EE was entering the plant oil house when a red wasp stung EE on the bridge of the nose. EE immediately began to swell. We transported EE to the local hospital where EE received steroids and medicine to counter inflammation through an IV. EE was released to return home soon after the swelling started going down. We are now aware of EE's allergy to certain insect stings.

Employee was finishing up last shift. EE was passing bricks to another crew that was at the same job location. They were working along with another crew to finish the rim on the kiln. The other crew was not attaching the bricks correctly and the bricks fell from the rim and some bricks landed on Employee's left finger and it broke it.

Checking cooler grates for #3 kiln by opening access door at the same moment a puffing phenomenon related to fuel mix occurred, blowing fine dust under low pressure out this opening some of the dust got around glasses and irritated eyes ee reported this at about 11:00 AM no lost time or restrictions returned to work same day with sulfacetamide 10% Ophthalmic solution.

EE was operating front end loader when he felt slight discomfort in chest & slight numbness in lt arm. He contacted supervisor & removed himself from loader. Ambulance was called for him to be transported to med. facilities. CPR was not performed, AED was not used & he remained conscious. Tests were ran & were negative for heart attack. He spent 1 night in the hospital

The gate controlling feed to the roller mill was being closed using a pipe wrench on a shaft fitted with a nut. Resistance was significant requiring downward pressure. Either resistance ended without warning resulting in head striking structure or grip was lost on wrench handle allowing it to rebound striking forehead resulting in (9) stitches.

Cutting bolts with torch to facilitate replacement of drive motor bases on screen when torch being re-positioned for the task brushed by left hand burning area of hand where thumb and hand are joined resulting in a second degree burn that has been seen by a Doctor several times and continues to be treated with Silver Sulfadiazine...free to continue normal job routine as of Monday

Cleaning out screw conveyor by pushing material down an opening when pushing too hard resulted in sharp shooter shovel penetrating too deep and employee jerked back on the handle causing the right little finger to strike the flight resulting in a bruise relieved by drilling a tiny hole in the nail to relieve pressure and three days light duty no lost time.

EE was separating sheets of 1/4" steel enough to get forklift forks between them. While pressing down on a 36" pry bar to obtain initial separation he strained his lower back. ER doctor instructions included no lifting until seen by Company doctor the following week. Meds to counter inflammation were also prescribed. Returned to normal activity 12/16/14 with no lost time.

Operator was performing a pre-start inspection of a Caterpillar 740D haul truck when the small lid on the left front fender that covers the master switch and the switch that raises and lowers the hood came down mashing his left middle finger resulting in a fracture of the finger tip...operator returned to work after treatment with no lost time or restrictions

saddle swung off pallet smashing hand, ring finger swelled...reported to superintendent who ask if he was injured and EE replied not now...there was no further discussion regarding finger until Monday...x-rays taken Monday 07/28 revealed no fracture but osteoarthritis...the nurse practitioner placed index and ring finger in a splint and place EE on restricted duty until 08/06

Foreman and Hydrator operator were in lunchroom when they herd a "pop". In seconds they were outside to investigate. The area near the Hydrate MCC and bag storage area were on fire, and the fire was growing. A brief attempt to control with extinguishers was abandoned and 911 was activated. No injury.

flat steel on forklift needed to be moved outward from the vertical portion of the forks...another worker moved his side first...this motion included a slight counter clockwise rotation...opposite side also moved with the rear corner sandwiching right index finger against the vertical portion of the fork...laceration to the right upper index finger...(4) stitches

Rotating a cone crusher bowl drive gearbox with chain from come-a-long when hook mount failed striking upper legs with chain causing fall onto deck...upper thigh was brused...didn't want to go to doctor...tuesday, back/hip pain resulted in Doctor visit/X-rays...perscription anti-inflamatory/pain meds and three days restricted duty resulted in this filling...he's fine

Employee was bagging 10 lb bags of hydrate when bag burst. Employee was wearing safety glasses, but some hydrate entered area behind glasses. Result was irritation to both eyes with the right side the worst. On site washing using water and eye wash plus several daily trips to eye doctor resolved the situation ending with two days restricted duty, but no lost time.
