THE INDIVIDUAL WAS GETTING ON A LADDER FROM THE TOP OF A 16' HIGH FRP TANK AND FELL. HE WAS NOT "TIED-OFF" AND NEITHER WAS THE LADDER. SERIOUS HEAD INJURIES AND BROKEN RIGHT ARM.
