Working to change the seal on an oil leak on the screen deck. Cut a hole with torch on Seco deck. Used a come-a-long to take the counter balance off. A spark must have gone into bottom of screen deck & smoldered. Flared up at 9:30.

Showing all 6Vendor ee was moving tires from a loader. He was using an excavator to remove tires. EE & another ee (plant mgr) had discussed need to put blocks under loader to crib it up. Loader moved & counter weight hit him in leg/side. 911 called
A ROCK FELL FROM THE DECK ABOVE DURING THE CRUSHING OPERATION AND HIT EE IN THE HARD HAT & THEN BOUNCED OFF HIS RIGHT SHOULDER. HE GRABBED FOR HIS HARD HAT, SPUN AROUND, HIT THE LEFT SIDE OF HIS HEAD ON THE STEEL I BEAM & FELL TO THE DECK.
WHILE GRINDING ON PLATE HE MOVED HIS HAND AND CUT FINGER ON STEEL PLATE.
HE TWISTED HIS LEFT KNEE WHILE PULLING ON A PIPE. HIS FOOT GOT STUCK IN THE MUD.
WHILE LIFTING A GAS CAN ONTO THE DRAGLINE, HE FELT A STABBING PAIN IN HIS LOWER BACK.
