While cleaning the floor in the SAG mil, employee attempted to throw a 5-inch grinding ball into an elevated ball bin. The ball came back down & struck employee's left middle finger as the were holding onto the lower part of the structure. Impact caught the finger between the grinding ball & the structure resulting in a laceration requiring stitches and a fracture of the finger.

As part of a routine inspection of a cyclone, the overflow pipe was removed & being reinstalled. The pipe was suspended by an overhead crane; another mechanic was pushing on the pipe to align the bolts. The pipe hung up, causing it to bounce. When it bounced, the nylon strap broke, & pipe fell, striking the mechanic's right hand between a pry bar & the overflow pipe flange.

Mechanic was working overhead installing a starter on a dozer when the screwdriver slipped from the mechanic's grip and fell striking the mechanic in the tooth. When the mechanic went into the dentist on 10/1/24 it was determined that the tooth was cracked and needed repair.

While standing and tightening the brake bleeder valve on a haul truck, the tech lost balance. The tech reactively stuck a hand out to catch self from falling and hit a knuckle on the accumulator bracket which caused a laceration

Employee set down trusses EE moved w/telehandler & was removing the slings when felt a pop in EE' arm. The next day examined by PA who set lifting restrictions. Employee was off work for holidays and returned to work on 1/5-1/8 w/restrictions. On 1/9 MRI showed torn muscle requiring surgery. Surgery was performed 1/11/24 @ which time injury changed from Restricted Duty to Lost Tim.

Employee was removing an air starter motor from a 785 CAT haul truck, when oil started pouring out of the starter mount. Employee quickly leaned to the left, while still holding the air starter, and smashed left ring finger between the starter motor and the truck.

Employee was lifting rubber mill liner segments off forklift & placing on liner machine to install inside the mill. Employee felt EE's forearm pop. Employee notified supervisor & was taken to ER. Employee was released to return to work w/further exams scheduled later. MRI results showed torn bicep requiring surgery. Employee missed first scheduled shift on 6/1/23

Mill Maintenance Mechanic was rebuilding a 12" Knife Valve. While re-aligning the valve plate, the piston slipped down and impacted EE's left thumb causing a laceration that required 4 stitches.

A mechanic disconnected a hydraulic hose from the final drive on a CAT 785 haul truck and placed one end up on the frame. As EE was putting plastic in place to cover the final drive, EE bumped the hose. It came off the frame and struck EE in the mouth. The impact from the end of the hydraulic hose cracked EE's upper right canine tooth.

While repairing A/C on a 777 CAT Water Truck a mechanic reached down between seats to check air flow, as EE was reaching down EE stumbled and EE's fingers entered the A/C intake and contacted the fan. Mechanic was taken to ER where finger tip was stitched up.

Maintenance was replacing pads on a dozer track. While trying to position the track, one worker struck the track with a hammer. A piece broke off & struck another mechanic in the leg. A metal shard embedded in the leg above the knee. After attempting to remove the shard, MDs left it in place, mechanic was treated w/antibiotics & returned to work on next shift.

On 1/15/2020 a mine mechanic slipped on the ice as the mechanic stepped out of a pickup. Initial medical diagnosis was a slight knee strain and the mechanic was allowed to return to work without restrictions. On 1/22/2020 the mechanic was seen by another doctor and it was determined that an MRI was needed for possible surgery and the mechanic was not allowed to return to work.

On 12/26/19 warehouse-person picked up a box and was putting it on a cart and felt a pain in back. This was reported to supervisor 12/30/19 because pain continued. Upon reporting employee was taken to local clinic. Diagnosis was a back strain & restricted for 2 weeks to lifting <10 lbs. Employee was placed on restricted duty.

Employee was removing an 8' section of 8' poly pipe from an elevated position on the reject stacker at the SAG mill. The section of pipe dropped a short distance while EE was trying to support it with the arm overextending EE's right arm tearing/detaching the bicep tendon. Results of 6/7/18 MRI revealed the surgery would be necessary.

Employee developed a skin rash/irritation after contacting epoxy on the skin of chin and wrist. Initial contact to the skin occurred on 5/11/18, employee noticed rash a couple of days later. Eight days after the initial contact the employee reported the illness to the supervisor on 5/19/18 and sought medical attention.

Employee was changing bit on blast hole drill. When EE reached to straighten the bit to connect it to the drill steel EE hit right little finger on a sharp edge of the breakout wrench. The sharp edge cut through the glove and lacerated EE's finger. The laceration required 4 stitches.

Employee needed to reposition the bit basket in the drill deck. Employee bent over and grabbed the bit basket with the left hand and pulled up on the bit basket. Employee felt a pop and an "electrical shock" sensation go up the left forearm. Injury to tendon in left forearm.

Employee was assisting a mechanic in moving final planetary out of a press. Employee held the hook on the lifting sling while the mechanic took up the slack in the sling with overhead crane. When the slack came out of sling employee's left index finger was caught between the hook assembly & planetary housing. This resulted in lacerating of left index finger requiring 5 stitches

Employee had finished lowering boosters down a blast hole and turned to walk to the next blast hole when he tripped over a rock and fell. The workers compensation insurance has been investigating this case to determine if the injury is work related. On 02/09/2015 we received notice that this incident is determined to be work related.

Employee stepped back off of a 2 foot step ladder and lost his balance causing him to fall backwards. The employee instinctively stuck out his right hand to break the fall and struck a tool cart that was placed in the area. The impact caused a laceration to the palm of the right hand. Employee did not fall all the way to the floor.

Employee was helping replace a section of 14" diameter by 55 long Drisco pipe that ran out through a wall and down into a surge tank. One bolt had been installed in the flange. The pipe was being rotated to make alignment of the holes. The pipe turned and rolled around and pinched employee's right middle finger between a 2" steel water line and the Drisco pipe.

Employee needed to repair a broken wire on a D-11 dozer that was located under cab floor. As employee was in this area he pushed against a hydraulic line, this caused dirt to fall from the hydraulic line onto his forehead and down on the inside of his safety glasses. Some foreign material got into the employee's right eye.

Two employees were working between two buildings. While facing one building Ice slid off the roof behind and struck one of the employees on the back of right shoulder. The uninjured employee stated they had looked at the roof edge and saw no overhanging ice or water dripping and they made the determination that the area was safe to work in.

Employee, an SOS contract employee working at Simplot Phosphates, was raising the feed pipe. She put her left hand on the block of the crane to steady it. As the crane block was coming up through the chute it pinched employee's left ring finger between the feed chute and the crane block.

Employee was helping to change lifters in a grinding mill. Nuts, sealers, washers and cones were removed from bolts. Employee was driving the old lifter out using a sledge hammer hitting the end of the bolts. As she hit the end of a bolt the hammer glanced off the bolt, her finger was caught between the hammer handle and the bolt.

Employee was helping perform maintenance on a conveyor belt. He put some tools in a service truck and was returning to get some more tools to return them to the tool room. As he was returning to the conveyor, he slipped and fell. Employee did not report the incident until the next day 05/20/2010.

Employee was switching a sealed sulfuric acid injection system, a rupture occurred in the line exposing her to a sulfuric spray. First aid was given by a safety shower for 20 min. Employee was transported to the hospital where first aid treatment was administered by a Dr. This injury is FIRST AID TREATMENT ONLY. Should medical treatment be needed later, report will be amended.

EE was arc gouging 09/05/2009. He said about 9:00PM his right eye felt like there was something it. About Midnight he woke with tearing and pain in his right eye and went to the ER. He did not contact his supervisor because he didn't think it was work related until the ER Dr. told him his eye suffered from arc flash. EE reported the incident by phone 09/06/2009.

Employee was holding a box of clean rags and was going to cut the side of the box with utility knife to open it so the rags could be removed from the box and placed into a "Clean Rag" barrel. The box was on its edge and employee was holding the box with her left hand and cutting with her right hand, when the knife slipped and cut the backside of her left hand.

Employee was replacing the A frame on a pickup truck. He was attempting to remove a bolt which was so tight that an extension was needed. As employee was pulling with both hands on the extension the bolt came free suddenly and the employee felt a pain in his left shoulder/neck area. The location of the bolt prohibits the use of an air wrench or longer extension.

Employee was changing picks on the Feeder Breaker; after he climbed out of the hopper (using the ladder) as he stepped from the ladder to the platform he felt a pain in his left leg just behind the knee. ER doctor released employee to work no restrictions 10/08/2008. Follow-up by another Dr. on 10/13/2008 he was placed on restricted duty.

Employee was in the process of installing a one inch air line to a new compressor. He was kneeling on his right knee tightening a fitting with a pipe wrench. As he pulled on the pipe wrench he felt pain in his right knee.

Employee was using a hydro-blaster to unplug a sanded hydrosizer. The hydro-blaster removed most of the sand that plugged the discharge. Employee pulled the hydro-blaster hose out to realign the nozzle with the discharge line. As he repositioned his hand on the hose, the discharge struck the outside edge of his left forefinger cutting his finger.

Taking bucket teeth off 992 loader, mechanic hit pin with hammer and a piece came off and hit wheeler mechanic on left forearm. The piece cut one vein and nicked the tendon.

On 05-11-2008 employee reported to his supervisor that he noticed an irritation in his left eye, he thought it was from the work he was doing the day before (05-10-2008). Employee stated he had been grinding and when he removed his grinding goggles he felt something get into his left eye. Employee was taken to see a doctor.

On 3/10/08 employee reported to his supervisor that he noticed an irritation in his right eye 3/6/08. 3/7/08, he went to see a Dr. Dr removed a piece of metal from his right eye. Employee stated that he doesn't know for sure when incident occurred he thinks it was related to the work he was doing on 3/5/08. Employee was working in the mine shop area using a torch and a grinder.

On 12/31/07 Employee was pulling and pushing a head mechanism on a drill press across discharge manifold flanges. This was done so that the flange faces would be the same length; he worked at this task for about 3 hrs. Employee reported he did not have any pain or soreness until the next day. On 01/02/08 he came to work and reported that he felt like he needed to see a doctor.

Employee was changing the teeth on the shovel bucket, teeth keeper pins had been removed. He was using a 10 pound sledge hammer to drive the old teeth off of the adapters. When he hit one of the teeth with the sledge hammer a small metal fragment (approximately 7 mm by 4 mm paper thin) chipped off of the tooth and lodged in his left forearm about six inches below the elbow.
