Air suspension on seat not working. Miner got a jar when EE hit a bump. Lumbar strain right sciatica. Light duty only. No lifting, climbing, limited reaching. Miner is off work as no light duty work is available and therapy is recommended by the doctor.
Miner was shoveling under a feed hopper and reported pain. Miner was told by a doctor there's a hernia and will require surgery to correct the problem. Miner will be off approximately 6 weeks.
Employee was cutting conveyor belting with a utility knife. Cutting action was towards him. The knife slipped cutting his leg above the right knee. Employee required 7 stitches & antibiotics.
EE was kneeling for extended time while welding on a conveyor tail pully, this caused bursitis in his knee.
EE WAS STANDING BY THE GRAVEL PLANT WITH HARD HAT AND SAFETY GLASSES ON. A ROCK THE SIZE OF A BASEBALL CAME OFF THE SCREEN AND HIT HIM ON THE HEAD (HARD HAT). STAYED AT WORK THE REST OF THE DA Y, AND HAS JUST NOW A MONTH LATER WENT TO THE DOCTORS.
THE CONVEYER WAS BEING LIFTED WITH THE LOADER TO BE PLACED ON TO CONVEYER STANDS. THE CONVEYER STANDS WERE NOT IN THE PROPER PLACE, AS THE CONVEYER WAS LOWERED IT SHIFTED CAUSING HIS ARM TO B E PINCHED BETWEEN THE PLANT AND THE CONVEYER. HEPLACED HIS ARM ON THE CONVEYER AS IT WAS BEING LOWERED.
EE WAS TESTING THE ELECTRICAL CIRCUITS WHEN AN ARC OCCURRED.
