Employee was assisting in removing a conveyor belt from a chute when a chain shifted and caught employee's finger between a steel beam and the chain. Employee sought medical attention and was diagnosed with a laceration and a finger fracture and is currently losing time. *Middle finger was amputated at the pip joint on 6/22/2021.

WHILE EXITING ROCK TRUCK EMPLOYEE LOST GRIP WITH LEFT HAND WHILE RIGHT HAND WAS MOVING DOWN TO NEXT HAND RAIL. THIS CAUSED EMPLOYEE TO LOSE BALANCE AND FALL TO GROUND. EMPLOYEE LANDED ON LEFT SHOULDER.
Hammering pins to remove teeth on excavator piece of metal chipped off hammer, embedded in skin, lower right forearm. EE was seen by dr on date of injury. 1st aid was provided (cleaned area). When piece of metal did not work its way out on its own the decision was made to surgically remove on 7/5. This is being reported now, based on the medical procedure done on 7/5/16.
Picking up rock out of 57 chute at secondary crusher and a rock shifted and hit employees hand cutting index finger on right hand.
Employee was walking to trailer to get tools. He walked under a conveyor but did not stoop down far enough, and as a result bumped his face on the conveyor.
Employee was removing a hammer from drill rotary head when the hammer slipped and pinched his right pinky finger.
Employee was sitting at the bottom of a slope. Another employee was sitting at top of slope. When he stood a rock approximately 3.5" diameter rolled down slope and hit employee in the back. Slope was approx. 7-9 ft.
Employee picked up wheel barrow and felt pain in lower back.
When exiting water truck, twisted right knee.
Employee was using sledge hammer to drive plate into the FMC feeder, hit finger with hammer (index finger, left hand). Employee required sutures to close cut. No missed or restricted days.
Employee was installing a grease fitting on the pugmill. He was in a squatting position with his left knee on the ground. When he stood up his knee "popped."
Employee lifted a light generator to attach to the ball on a truck. When he set the generator down, he felt something pull in his back.
Employee was working under dry screen and a small rock bounced off the screen and struck employee in the back. Employee was taken to medical clinic. Has a bruise on back. Doctor took employee out of work for two days, August 19th and 20th. Employee returned to work today, Thursday, August 21st.
Employee was turning on the water on the wet screen. As he turned to leave the wet screen he hit his head on a ladder causing him to fall. The fall broke his leg in two places. An ambulance was called and transported him to the emergency room at Stringfellow Hospitol in Anniston, Alabama. Employee is scheduled for surgery later today.
MAN WAS HELPING WITH REPAIRS TO CONE CRUSHER ANDHE JUST DID NOT WATCH WHAT HE WAS DOING AND STEPPED BACKWARDS FROM THE SIDE OF THE CRUSHER AND STEPPED RIGHT OFF THE MAIN FRAME OF THE CRUSHER A ND FELL OFF TO THE CONCRETE PAD UNDER THE CRUSHER. HE FELL 6 1/2 FEET.
