Initial diagnosis on 09/10/2014 was contusion, prescribed muscle relaxors and to take over the counter pain meds. Follow up visit on 09/12/2014 with Dr., diagnosis is likely a hernia, appointment for surgery scheduled for 9/15/2014 at 9:30 AM.

WHILE ATTEMPTING TO OPEN A FROZEN AIR BRAKE LINE ONE OF THE LINES WAS REMOVED WHILE UNDER PRESSURE AND SPRAYED AIR GUARD LIQUID INTO FACE. ALTHOUGH WEARING SAFETY GLASSES,SOME OF THE LIQUID MA NAGED TO GET UNDER HIS CONTACT LENS CAUSING A BURN ON HIS EYE WHICH DIDN'T SHOW UP UNTIL THE FOLLOWING DAY.
