Miner was changing the teeth on an excavator bucket (Cat 390 #1-15). The miner was using a hammer to strike the retaining pin on the tooth when a piece of steel broke and hit the miner in EE's right leg causing a laceration.

Showing all 8Miner was climbing down from feeder and stepped on berm. Miner rolled right ankle. Medical evaluation revealed fracture of distal fibula.
Miner was repairing the wash screw tub and was carrying a fabricated part to install. EE lost footing as EE walked under the tub and struck head on the flange of a nearby water pipe. EE received a laceration above the right eye. EE was taken to the clinic to receive treatment.
Employee (plant repairman) was removing welding hose from truck and cut thumb on the body of the truck.
At approx 2:39 PM under cloudy cool conditions, EE had just completed repairs to vibratory screen (S-9) and had descended the catwalk to the ground. EE was preparing to enter the passenger side of the repair truck cab when EE succumbed to a seizure. Paramedics were called immediately and removed EE to seek medical attention.
Employee seems to have fallen out of his loader following fueling. He does not remember what happened, but evidence at the scene indicates that the nozzle was left in the tank as he drove away. Injuries to the employee include broken ankle, shoulder, ribs, nose, and facial fractures.
Employee was climbing off a conveyor frame, mis-stepped onto an electrical cord, and twisted knee.
EMPLOYEE WAS REMOVING LOADER FROM BUCKET USING A SLEDGE HAMMER UPON STRIKING TOOTH A SLIVER OF METAL BROKE OFF AND PENETRATED LOWER LEG.
