stumbled while taking out trash.
Worker was found unconscious in the restroom by a coworker and taken by ambulance to Medical Center. Worker has no memory of what led to the incident. They suffered an injury to their head and arm when they fell.
Employee was fueling the loader when fuel splashed in EE's face, going into EE's eye
Blocking material slipped out from under track hoe bucket, allowing bucket to roll forward and pinched heel on right foot between bucket and garage floor.

EE was cleaning conveyor belt with rag, rag caught by pulley, taking left hand along conveyor.
Employee was tapering off 4 x 3 1/4" bar with grinder making a pin for excavating bucket when hand slipped, resulting in 2 inch laceration on inside of left wrist.
3 men repairing conveyor belt-one man had a crowbar lifting the pulley up while EE had his hand underneath repairing belt-crowbar slipped causing EE's left thumb to be crushed between bearing and adjuster plate.
USING A CUTTING TORCH IN GARAGE NEAR A BUCKET OF UNKNOWN FLAMMABLE MATERIAL. SPARKS FROM TORCH IGNITED IT, RESULTING IN AN EXPLOSION AND FIRE.
