Employee was in a manlift approximately 2 feet off of the ground cutting off a piece of metal. Once the cut was completed the employee pushed the cut piece away from them and employee said something hung their glove and pulled them out of the manlift. Employee landed on their shoulder separating it

Showing all 5EE was removing built up dirt and rock debris from a piece of equipment. While using a pick a large piece of rock/dirt fell on his leg.
Not sure of full conditions, because after finding a note from ER in the truck stating his return to work date 4/17/12. The investigation was called off, by MSHA. The truck ran on to a berm and flipped on its side.
An articulating truck driver was operating his truck when the left front tire blew out. The force blew upward toward the cab and the man hit his ankle on the dashboard breaking a bone in his left ankle.
Climbed up on side of crusher while it was operating, caught arm in head pulley. It dismembered arm at elbow.
