Sawyer/Park narrow belt saw;small roughback grazed back/left leg.Front roughback/slabs removed.Sawyer didn't follow procedure w/back roughback;thus his injury.Went to foreman/examined his leg w/him;back to work/no concern.Next day his ankle was hurting,taken to doctor & told to take off the day/stay off it over weekend.No breakage,only bruising.Off wk,w/pay,for 4days.Back:6/30/2011

EMPLOYEE STEPPED DOWN FROM A LOADER INTO A HOLE IN THE GROUND CAUSING HIM TO TWIST HIS ANKLE AND FALL. CONTRIBUTING FACTORS WERE THAT THE EMPLOYEE DID NOT CHECK THE GROUND CONDITION BEFORE STE PPING OFF THE MACHINE AND AN UNEVEN SURFACE CONDITION. INJURY INVOLVES SPRAINED ANKLE WITH TORN LIGAMENTS.
