EE WAS TIGHTENING A NUT ON TOP OF THE CORE OR THE CRUSHER. HE HAD ATTACHED A LARGE WRENCH TO THE NUT AND WAS STANDING ON TOP OF THE WRENCH WHILE SWINGING A SLEDGE HAMMER AT THE END OF THE WREN CH TO TIGHTEN THE NUT. HE SWUNG AND MISSED AND THE MEMENTUM OF HIS SWING CAUSED HIM TO LOSE HIS BALANCE AND FALL HE FELL ON TOP OF THE BOLTS THAT HOLD THE CRUSHER DOWN.

TWO PULLEYS ON THE CRUSHER HAD JAMMED & INJURED WAS TRYING TO FORCE THEM BACK INTO OPERATION. HE HIT ONE & IT STARTED. THEN HE USED A WRENCH TO TRY TO FORCE THE OTHER INTO OPERATION. HE WAS SUCCESSFUL BUT AS THE PULLEY BEGAN OPERATION THE BELT ON THE PULLEY PULLED THE WRENCH INTO CRUSHER ALONG WITH INJURED'S HAND. HIS HAND WAS LACERATED AND SQUEEZED BUT NO BONES BROKEN.

EMPLOYEE WAS GREASING A BEARING ON THE CRUSHER WHEN HE LOST HIS BALANCE AND FELL APPROXIMATELY 10 FEET TO THE GROUND.
