The miner climbed the ladder on the side of the aggregate feeder to dislodge some stuck material. The material dislodged causing the bin to shift. Which caused the miner to fall off of the ladder, injuring EE's left shoulder and left index finger. EE received stiches to the finger.

Showing all 5The miner was changing a bearing on the head pulley of a conveyor. When EE loosened the pillow block bearing the shaft popped up striking EE in the palm of EE's left hand causing a laceration which required stitches.
The miner was lifting a piece of steel for one of the conveyors in the quarry. While lifting the piece of steel they felt a pop in right arm/shoulder. The information about this injury was obtained through reports taken by the safety director at the time of the injury. Who was no longer employed at Rogers Ready Mix & Materials as 10/14/2019
EMPLOYEE WAS CHANGING A SCREEN, HIS BODY JERKED INTO SAFETY CABLE BREAKING CABLE FALLING INTO HOPPER OF CONVEYER ON HIS BACK.
THE PLANT WAS SHUT DOWN. THE EMPLOYEE WAS REMOVING A BOLT TO REMOVE A GUARD WHEN HE LIFTED HIS HEAD UP STRIKING HIS HEAD ON THE BOTTOM LEFT CORNER OF THE GUARD RESULTING IN THE LACERATION.
