Employee claims left leg slipped behind them while exiting the restroom. Employee claims left knee made contact with the floor. Employee stated that there were no physical hazards present. Employee did not seek medical attention until 12/24/18, and provided a work restriction form on 12/26/2018.

Miner was attempting to pull a piece of conveyor skirting from a chute when EE lost balance and fell backwards onto the catwalk. Miner sustained a lacerations to EE's left hand requiring 4 sutures to close.

Miner reported 03/15/2018 that they had sustained a work related injury to the back. The miner was moving/lifting a 4" rubber flex hose for the dust conveyance system in order to free a clog on the morning of 03/14/2018, when they noticed they were experiencing back pain.

Miner was placing tools and materials in the back of a maintenance truck when EE felt something in EE's boot. When EE took another step a nail protruded through EE's boot sole and into EE's right foot.

Injured employee had just completed rigging a load on the 300 ton Link belt crane. EE made eye contact with the operator and asked if EE could come and talk to EE. While up on the deck talking to the Crane operator EE fainted and fell to ground and broke left clavicle. Employee was taken to Hospital where EE was found dehydrated and taking 3 meds for blood pressure.

Miner was using a pipe wrench to loosen spray nozzles on wet screen. When the threads loosened, the miner's right hand came into contact with the spray bar above causing a laceration requiring stitches to close. Miner was released to full duty.

Miner states their knee buckled at ~5:30am while dismounting CAT 775 haul truck at end of night shift on 06/11/2017. Miner did not report the incident and went home. Miner called their supervisor at ~4:30pm on 06/11/2017 to report the right knee was swollen. Miner went to local ER diagnosed with knee strain. Miner was released back to work with restriction 06/12/2017.

EE was trying to break frozen chunk of sand that was lodged in feeder opening. When tried to hit chunk with sledge hammer, hammer struck steel brace, bounced towards EE causing the end of hammer handle to make contact to the EE left leg near his knee. EE was taken to the ER for X-rays. No broken bones were found. EE returned to work. EE saw dr 03/02/15, given restrictions.

Miner bent over to turn off 2" water valve and felt pinch in lower back. 1st Dr visit 10/30/14 released w/o restriction, 2nd Dr visit 12/11/14 released w/o restriction, 3rd visit 01/26/15 given work restriction and Rx medication that will not allow to work.

Miner was changing the shaft and bearings on the head pulley of the 40/70 bucket elevator. He and another miner were trying to place a pry bar between the belt and the pulley to position the belt. The miner' s finger was pinched between the pry bar and the belt.

Mechanic changing cutting edges on 938K loader. Two maintenance employees stopped and started to help. While installing last bolt on left side of cutting edge, employee put palm on bolt to hold in place while mechanic tightened bolt. Bolt came loose and spun causing cut through glove and employee's hand.

Driller and Assistant were pulling casing. The assistant turned the casing loose. The driller pulled casing up, the casing stuck and then released causing the casing to jerk in an upward motion hitting the driller assistant in the mouth.

Employee had the rear of the fender un-bolted on a 938K (SWL02604)and laying on the tire and had one bolt holding the front of the fender. Then it slipped and fell with my left hand under it. It appears that it was hung up on open door latchback linkage and then slipped off. There was a sharp point that cut the little finger on left hand.

Company was not notified of specialist 06/12/13 until 08/04/14. Injury meets reportable status on 06/12/14.

On 03/02/2014 miner reported that his eye had become irritated at the end of his shift while picking up for the day. High Easterly winds prevailed most of the day on 03/01/2014. EE taken ER,examined,given antibiotic,released w/o restriction. 09/11/2013 EE contacted mgmt via phone complaining of drainage/dizziness. 10/04/13 EE saw ENT specialist. Examined, given antibiotic. Testing revealed perforated ear drum. 01/07/2014 EE underwent surgical procedure to repair ear drum.

EE was installing belt wiper on BC02 to divert stone. EE used his personal pocket knife to "mark" the belting in order to punch holes in the belting for mounting. While EE was "marking" the belt, his knife closed, causing a laceration to his right ring finger. EE was seen at local hospital, required 6 stitches to close wound, and released without restrictions.

Miner was dismounting Bobcat 130 skid-steer loader when he lost his footing. Miner attempted to catch himself when he heard his shoulder pop, and he went to the ground. Miner was treated and released for a dislocated shoulder at the Heart of Texas Memorial Hospital ER.

EE injured his right elbow on a handrail while working on conveyor belt 6 (CV-6) on 082012 at 8:00 PM. EE did not consider the event as anything major and continued working throughout his entire shift. The next day (roughly around 12:00 PM), EE noticed that his right elbow was starting to swell.

On Feb 14th, 2012 at 9:00pm, EE reported that his back was sore from shoveling sand around the CV3 tail pulley system. The reason ee was shoveling was do to spillage at CV3 as a result of carry-back on the CV1 belt. On Feb 15th, 2012 at 10:26am, ee was seen by a doctor and was given restrictive duty/activity (no shoveling/no lifting over 20lbs).

Miner was loading 6" PVC pipe into the back of a plant pick-up truck when his right hand moved over a burr on the pipe. The miner was not wearing gloves and sustained a laceration to his right hand. The miner was treated at Knox County Hospital where he received a tetanus vaccination and 7 stitches to close the would. He was released to full duty.

Changing out oxygen bottle. Had put up empty bottle and was moving full bottle, he picked up the bottle to go over some welding leeds and his foot got caught in welding leed. He balanced and fell holding bottle which caused his right hand between the ground and bottle.

Employee called the morning of Wednesday, January 5, 2011, said his ear was badly swollen, that he needed to see doctor. He stated around 9-10am on Tuesday, January 4, 2011, he noticed pain to ear while working near dam generator. Doctor suspects the employee was victim of a spider bite.

Employee was removing forks from a rough terrain forklift. As the pin securing the forks to the mast released, the forks tilted back to the mast catching his middle, right finger between the fork and the mounting. Employee received 5 stitches, and was released to return to work w/out restrictions.

Employee reported injury on 07/19/2010. States that he was digging hard bank with loader on his last shift, 07/16/2010, and had pain in lower back. Employee saw doctor and given restrictions effective 07/20/2010. Employee stated that he was not sure exactly what caused injury. The air seat in the loader was/is in proper working condition.

The victim was a truck driver for John R. Trucking Co. He was found unresponsive in the cab of his truck. 911 was called, CPR was administered by 2 EE's and an AED was brought to the scene. The AED was attached and instructions were followed. Hopewell TWP. EMT arrived shortly after and took over.
