Quality control technician was adjusting the forks on a Gradall forklift & placed right hand between the back of the fork and the back of the attachment, resulting in a laceration to the right ring finger and small finger.

This report is being submitted at MSHA's request on 2/14/2020 as a result of an incident on 2/7/2020. A third party hauler suffered a medical emergency at the mine entrance. Emergency services were called and hauler was taken offsite by an ambulance.

On 11/7/19 the employee received stitches to close a laceration. While hosing the catwalk near the feeder, the feeder became clogged, and a rock fell out striking the employee on EE's hand resulting in a laceration to the right index finger.

On 10/8/19, the employee was descending excavator when EE's left knee buckled and EE fell to the ground. The employee was treated at the Company clinic where EE was diagnosed with a left knee sprain and a right shoulder strain, placed on restricted duty, and prescribed physical therapy

Employee requested medical treatment on 2/21/18 and was prescribed work restrictions by the occupational physician. The employee alleged that ee stepped on a rock, felt two pops in L-knee and felt pain. This is an alleged, non-witnessed questionable event.

On 8/16/17, the employee received medical treatment for a right wrist fracture, was placed on restricted duty which was accommodated and subsequently lost time on 8/28/17. The employee's pant was caught on a fence post that was leaning against a truck while walking backward carrying a fence with a co-worker. This caused EE to fall backward; EE used arm to break the fall.

On 12/11/15 the employee was taken to the company clinic, was prescribed physical therapy and released to full duty. The employee hopped over a water puddle and struck his head on the bottom of the sand screw tank. He was diagnosed with a neck sprain and shoulder strain. The employee reported the incident on 12/11/15.

The employee was taken to the Company clinic on 2/2/15, was provided wrist and elbow braces along with physical therapy. Employee alleged pain in L-shoulder, elbow, hand, wrist and neck from repeated opening of a window to issue tickets. She was diagnosed with L-shoulder sprain/strain, neck sprain, L-hand/wrist tenosynovitis, L-elbow/forearm synovitis, L-upper limb mononeuritis.

On 3/3/09 the employee was taken to the hospital and was admitted for more than 24 hours. While the employee was descending ladder on loader, his leg gave out and he fell approximately 4 feet resulting in R-leg and pelvis fractures. (Employee has an underlying medical condition which may have been a causal factor).

The employee hurt himself on 6/26/2007 but did not choose to go see a doctor until 7/13/2007.He was placed on restricted duty on 7/13/2007.The employee twisted R-knee while chasing water truck to get the operator's attention, which resulted in a contusion to R-knee/R-ankle.
