Was parked in dump truck next to the shop ( not inside gated pit areas) He stepped out of his truck and walked along edge of shop where his foot got caught in a piece of belting laying on the ground. His left foot got caught and he fell down on both knees.

Rock stuck in tailgate. Lifted tailgate with catch post. Tailgate fell and crushed top of right first finger.
Jumped over a mud puddle and felt a twing in his knee.
Employee and witness were removing an oversized rock which was wedged between two belts on the crushing plant. Employee was underneath trying to push rock up and witness was on top trying to pull rock. The rock slipped smashing employee's right thumb.
EE WAS PERFORMING PIT REPAIRS AS A PART OF HIS ASSISGNED DUTIES. HE TRIED TO LIFT A PIECE OF METAL, CAUSING THE INJURY TO HIS LEFT SHOULDER.
WHILE DESCENDING THE LADDER STEPS ON THE CAT 950 LOADER, EE SLIPPED, FALLING ABOUT 4' TO THE GROUND, AND LANDED ON HIS RIGHT SHOULDER. ALTHOUGH ACCIDENT OCCURRED IN MARCH 2001, IT WAS NOT REPO RTED BY EE TO MGMT. PERSONNEL UNTIL 5-31-01.
EMPLOYEE WAS PERFORMING REPAIR WORK TO TOP SOIL PLANT WHEN HE BANGED HIS LEFT KNEE ON THE CQATWALK/SCAFFOLDING.
