While employee was changing screens on washplant, his safety harness caught on spray bar. He slipped and strained his back.
Employee was driving haul truck when he hit a pocket of silt which jolted the truck. He experienced pain in lower back.
Employee was feeding crusher when going up ramp dropped a rock. When going down hit rock, jolting cab. Didn't feel stiffness until after work. Reported next morning to Leadman (10-06-04). Did not request medical attention from Company until 10-7-2004.
EMPLOYEE WAS CLEANING AROUND A CONVEYER BELT. AS HE WAS SHOVELING HIS THUMB WAS CAUGHT BETWEEN A TROUGHM ROLL AND THE ROLL FRAME. THE EMPLOYEE'S THUMB WAS CUT AND REQUIRED TWO STITCHES.
EE STATES THAT AFTER WORKING A SHIFT, THAT INCLUDED CHANGING TAKE-UP ROLLS, HIS BACK BEGAN TO HURT. EE DID NOT NOTICE ANY SPECIFIC INCIDENT THAT CAUSED AN INJURY. HIS BACK DID NOT BEGIN TO HUR T UNTIL AFTER THE EE HAD BEEN HOME FOR SEVERAL HOURS.
