Employee reported on 1/25/07 that his knee hurt (gave no reason why it hurt). He requested to go to a doctor on 1/26/07 and told the supervisor that he hurt his knee on 1/22/07 while coming down off a ladder.

EE was removing a stone (appox 90lbs) away from a crusher chain feeder tail adjustment. While moving the stone, another became dislodged, rolling and striking his foot.

INDIVIDUAL WAS TAKING OFF A BEARING SHAFT ASSEMBLY (WEIGHING 75 LBS.) FROM A TAKE-UP PULLEY IN THE SHOP. HIS RIGHT HAND SLIPPED OFF AND THE BEARING SHAFT ASSEMBLY FELL ON HIS LEFT HAND (RING FINGER). INJURY REQUIRED FIVE STITCHES - NO BROKEN BONES. EE RETURNED TO WORK THE SAME DAY.

EMPLOYEE WAS APPLYING BELT DRESSING TO BELT. HE THEN SIGNALED TO START THE BELT. THE TURN BUCKLE HOLDING GEAR REDUCER BROKE ALLOWING THE REDUCERAND MOTOR TO SPIN BACK AND STRIKE THE EMPLOYEE.

EE WAS ASSISTING DRILL OPERATOR CHANGE HAMMERS ON DRILL. TOOLS USED WERE THE HYDRAULIC OPERATED PIPE WRENCH AND A J WRENCH. EE PLACED J WRENCH ON THE DRILL STEEL SO THAT IT WOULD REST AGAINST THE MAST WHILE THE PIPE WRENCH TURNED THE HAMMERWHEN HAMMER BROKE LOOSE, THE J WRENCH KICKED BACK STRIKING THE EE IN THE RIBS.
