Employees were unclogging a baghouse and a hard hat fell into the screw at the bottom of the baghouse. The hard had was drawn into the screw and stopped at the rotary valve. They workers locked out the screw and valve and attempted to retrieve the hard had. When the hard hat came out, there was stored energy in the valve and it turned and caught the workers fingers.

Showing all 32Worker was cleaning the plant floor and encountered a guard that had fallen from a head pulley on a conveyor. EE attempted to replace the guard and the active conveyor caught it. EE's hand was smashed between the guard and adjacent steel casing.
While installing the bucket on a skid steer, the employee could not close the latch easily. EE utilized a pipe to gain leverage on the latch and was able to close it. After this, the pipe was jammed on the latch handle. When EE pulled the pipe from the latch, EE smashed EE's left thumb on the skid steers frame. The thumb has a laceration and an open fracture of the distal phalanx.
Worker was attempting to weld a short length of pipe to the bottom of a bin. EE tack welded the pipe in place in preparation for the weld. The tack weld failed and the pipe fell and the employees thumb was impacted between the pipe and adjacent angle iron. The impact resulted in a fracture to the left thumb.
Moving boxes and tripped over a strap on floor
Employee missed 2 steps and fell on EE's right hand.
After loading at Superior Pit and Mill location driver was performing an inter-trip inspection when EE stepped down off of the loading platform onto a rock and turned EE's ankle. Driver experienced immediate pain and the sensation of rubber bands popping.
Operator was using chain fall and reported sore muscles 4 days later.
Individual was performing maintenance on a bucket elevator and got their fingers crushed when the bucket elevator belt unexpectedly moved
Employee was pushing wheelbarrow when the employee felt burning sensation in stomach.
Repairing leak on pressure washer.
Employee exited cab of haul truck to wipe mirrors and windows, felt light headed falling forward, tried to catch self continued forward striking rail, scraping left forearm and felt pain in left shoulder.
Employee was picking up hitch end of welder. Welder hitch slipped, employee grabbed for hitch and felt pinch in right wrist.
EE was taking a measurement of conveyor belt. Tape measure got caught and ee reached to grab the tape and while doing so the edge cut his index finger. Required sutures to repair wound.
Employee was fueling service truck, when dismounting he missed a step and fell 3' to the pad. Landing on his butt.
Employee was carrying boxes to dispose of, when he tripped on one of the legs of the threader and hit his back on the treading machine.
After removing lock-outs from cone crusher disconnect, employee was descending steps from upper control room to base floor & slipped. May have hyper extended left knee.
Employee got right pinky finger caught in between V-belt & Sheeve, while replaceing V-belts on #2 M.B.H. fan. Amendment: Employee worked full duty till 4/7/11 when he went off work for surgery.
EE was working on #8 Screen Deck and slipped on Deck Platform, striking his left knee against railing.

WHILE REMOVING A SECTION OF HANDRAIL, EE WAS HOLDING HANDRAIL AND PULLING IT UPWARD WHILE OTHER EE WAS STRIKING HANDRAIL. EE THEN MOVED HAND AND OTHER EE STRUCK HAND WITH PRY BAR.
Employee was filling forklift propane bottles when he lifted a bottle and turned, twisting his back.
Employee was drilling a hole to mount a mirror on the cab of the loader and a piece of material embedded in the left eye.
EMPLOYEE WAS CUTTING & WELDING ON A TRAILER SPRING HANGER WEAR PLATE WHEN A PIECE OF SLAG FELL INTO RT. EYE.

While unloading truck, the wind blew perlite in left eye. Physician removed foreign object from eye.
EE WAS CLOSING A BOTTOM GATE ON A RAILCAR AND LATER OPENING A GATE ON TOP OF BIN #4 WHEN HE FELT PAIN IN HIS RIGHT SHOULDER.
TRUCK DRIVER LOADING BULK PERLITE ORE IS LOWERING THE LOAD OUT SOUT BY THE USE OF A CRANK OPERATED WINCH WITH CABLE CONTROLING THE SPOUT WHEN VRANK SPUN DURING SPOUT DOWN MOVEMENT AND THE TRUC K DRIVER ATTEMPTED TO STOP THE CRANK BY PLACING HIS HAND INTO THE CRANK MOVEMENT.
EE STATED THAT WHILE TRYING TO BREAK LOOSE SOME MATERIAL STUCK IN THE JAW CRUSHER, MATERIAL FELL, CAUSING THE BAR HE WAS USING TO FALL INTO THE CRUSHER JAW WHICH PINNED MOMENTARILY HIS FINGER BETWEEN THE BAR AND PART OF THE UNIT STRUCTURE.
THE EE STATED THAT WHILE BREAKING ROCKS WITH A SLEDGE HAMMER, A PIECE OF ROCK FLEW OFF AND STRUCK HIM IN THE LEFT LEG JUST BELOW THE KNEE. THE SKIN WAS NOT BROKEN, BUT THERE WAS NOTICEABLE SWE LLING AT THE INJURED AREA.
EMPLOYEE STATED THAT HE BELIEVES WHILE PREPARINGTHE IMPACT CRUSHER FOR SERVICE HE REINJURED HIS LOWER BACK. EMPLOYEE STATED THAT HE FELT NO PAIN OR DISCOMFORT UNTIL THAT EVENING WHEN GETTING R EADY TO LAY DOWN FOR THE NIGHT. EMPLOYEE ALSO STATED THAT HE BELIEVES IT IS FROM AN UNREPORTED INJURY SEVERAL YEARS AGO. SUSTAINED WHILE MOVING BAGS.
EE STATED THAT HE WAS USING A PRYBAR TO REMOVE A CONVEYOR OR ROLLER FROM ITS BRACKET. WHILE PUSHING DOWN ON THE PRYBAR, THE PRYBAR SLIPPED CAUSING HIM TO LOSE HIS BALANCE AND FALL TO THE CEMEN T FLOORING ON WHICH HE WAS STANDING.
