Removing bolt from engine panel. Bolt would not back out. Employee grabbed the other end of the bolt without looking and bumped the impact. A piece of metal that was on the bolt punctured the cut resistant glove and cut their ring finger.

The #1 belt conveyor stalled during normal operations, the injured EE and co-worker were told to check the problem out. The v-belt guard was removed. EE placed hand on the upper v-belt. The belt conveyor pressure released itself turning counter clockwise pulling EE's fingers into the pulley, severing parts of three fingers.

He was working on the radiator of the loader. His wrench slipped while taking radator cap off causing him to lose his balance and he fell approx. 4 ft. When he hit the ground he injured his pelvis.

He was weighing his truck. He went to get out of truck (they are instructed not to get out) and fell as he was getting out.

HE WAS WORKING NEAR ANOTHER EMPLOYEE THAT WAS WORKING ON A PIECE OF EQUIPMENT. EMPLOYEE SWUNG A PIECE OF PIPE AROUND & IT STRUCK INJURED EE ON THE LEFT ARM.

Working on a pump and reached across a hot part and burned right forearm.

EMPLOYEE TOUCHED HOT WIRE THAT HAD BEEN HEATED. WITHOUT GLOVES, HE BURNED HIS RIGHT INDEX FINGER.

EE WAS USING BURNING TORCH WHEN HIS SHIRT SLEEVE CAUGHT FIRE CAUSING MINOR BURN TO ARM.

EE WAS WORKING IN AREA WHERE WELDING WAS BEING DONE. RECEIVED FLASH BURN TO EYES.

EE WAS USING CROW BAR TO DISLODGE ROCK FROM HOPPER. BAR SLIPPED & MASHED FINGER.

EMPLOYEE WAS WORKING ON ROCK CRUSHER WHEN A PIECE OF METAL DROP ON HIS MIDDLE FINGER ON RIGHT HAND.

EE STEPPED FROM CONVEYOR BELT ON A ROCK AND TWISTED HIS LT. KNEE.

EE WAS SPLICING CONVEYOR BELT. WHEN HE STRUCK HIS THUMB WITH HAMMER EE HAD 4 OR 5 STITCHES IN HIS THUMB.

EMPLOYEE WAS LOADING ROCK WITH FRONT LOADER WHEN HE BRUISED HIS BACK, CAUSED FROM JARRING AND SHAKING OF LOADER.

EE WAS USING PORTAPOWER WHEN HIS THUMB WAS PINCH.

ROCK FELL FROM CONE FRAME, STRIKING EMPLOYEE IN BACK CAUSING BRUISE TO THE BACK.
