Light rain was falling, standing water, wet equipment, electrical wiring was color coded incorrectly and wired wrong in the junction box, the ground lead was wired into the buss bar and caused the equipment frame to become energized. Employee set up a metal ladder to inspect equipment and received electrical shock.

Employee was working under conveyor and grinding. He remembers his right eye being irritated at the time but did not report the incident until 7-23-07 since it had not cleared up on its own.

Employee fell from front end loader ladder while dismounting loader. He fell backwards and injured lower back.
