Employee was hanging 50lb empty bags on loading spout and felt a pop/pull in EE's right forearm. Massaged it for a while and continued working thru EE's shift. Came back the next day worked half a shift doing same job then changed jobs for 2nd half. Not sure of damage, was told to follow up with an orthopedic doctor.

Working on bend pulley on overlay conveyor, load shifted and caught maintenance workers index finger. EE was able to pull finger out of the glove, but the tip of the finger was bleeding and first aid did not stop it. Doctor visit confirmed tip of finger was fractured, but did not stitch or glue the finger. A splint was placed, no work restrictions.

An employee found a conveyor motor was turning but the gear reducer was not as the V-belt was slipping. The employee removed the guard and pulled the V-belt to add tension when the belt started and pulled ee's right index finger between the belt and pulley causing a laceration below the second knuckle resulting in 2 stitches and returning to work with no restrictions.

EE was caring a bag of sand on his shoulder. A wind gust blew a sand particle into his eye. The employee was wearing safety glasses as required. The employee did not report the incident until he returned to work his next shift (7/24/15). His eye was irritated. The doctor removed the foreign body and gave him a prescription for an antibiotic cream and he returned to work.

While hanging bags on a paper bag sealer in the Open mouth bagging area the employees right hand middle finger was pinched causing a laceration. No stitches were required. Employee was placed on limited use of the right hand.

Employee was operating the Bagging Machine when he noticed his hand was swollen. He put Ice on the hand. He went to the clinic to have it checked out. He was diagnosed with a Mild Strain to the Extenser Tendon. Employee indicated that he noticed his hand being swollen on the previous shift but failed to report it.

Employee was helping the lift truck operator lift a 6" x 25' rubber hose out of a storage rack when his right index finger got caught between the fork of the lift truck and a steel support on the rack. This caused a laceration requiring 23 stitches to close.

Employee was walking through the maintenance shop, another employee was grinding on a steel screen frame. The injured employee received grinding dust in his right eye under his safety glasses. The employee had the foreign body removed at the clinic and received prescription eye drops.

An employee was installing a hydraulic motor on a portable screening machine. Due to oily conditions, the motor slipped from the employees hands and when he attempted to catch the motor, his finger was pinched between the motor and the equipment frame.

On 1/21/09, Best Sand was notified through the BWC that Injured EE had been seen by a doctor due to numbness, tingling and pains in his hands and fingers. The doctors report listed the initial diagnosis as Bilateral Carpal Tunnel. He did not report this injury/illness to Best Sand management personnel.

The employee was using a spud bar attempting to dislodge jammed rocks from primary crusher jaws, when a large rock fell from top of pile, striking the bar causing it to swing and hit him in the right side of head. The employee was diagnosed with a slight fracture to the lower jaw bone.

A non-employee over the road truck driver claims that she slipped while climbing down the ladder of her air-can trailer and fell approx 6' to the lower level of her trailer. Non-employee suffered several scrapes and bruises and cracked ribs.

EE was climbing a rope ladder to exit a bin, lost his grip and fell about 20 feet. EMS was called and a lower access door was removed. After EMS exam EE walked out of the bin and was transported to the hospital for an exam. It was determined by a CT exam that EE had suffered a L3 transverse process fracture (small bone in lower back). He was released that night from hospital.

EE was installing windshield in 980G loader with with a helper. He was standing on the Z-bar linkage on the right side of the machine holding the windshield while helper was on the opposite side holding that side of the windshield. He said his foot slipped causing him to twist his back while attempting to not drop the windshield. *Determined to be a reoccurence of 2001 injury*

The employee was walking under an overhead conveyor inspecting the belt splice as it was moving along. The employee with head and eyes turned upward, walked into the tail pulley guard of another conveyor. Apon contact, the employee instinctly bent forward hitting his right front tooth on guard and knocking it completely out of its socket and also chipped it.

The employee was cutting out a plastic backing plate for a wet location electrical box. He held the plate in his left hand while using a utility knife with his right hand to cut out the tabs. The knife cut through the plastic tab and continued down and away from his body but contacted the tip of his left index finger all in one motion. The laceration needed four stitches to close.

The worker fell from the bag plant roof where he had been cleaning to the truck dock approximately 17 feet below. The specific explanations for being near the edge or for falling are unknown. Weather conditions and working surface believed to be non-contributory. There were no direct eyewitnesses to the fall. The fall resulted in the death of the worker. Investigation ongoing.

Employee was removing urethane buckets from an elevator. He was using a grinder to cut the bolts. The grinder wheel contacted the bucket enough to create fumes which he breathed. He later stated he did not feel well and went to the hospital where he was diagnosed with inhalation pneumonitis.

The injured employee was removing the bottom access panel of the rosin dryer elevator which had plugged up with wet sand. After removing the clips that held the panel on, EE removed the door. As he was doing this the upper panel pivoted downward because the bolts had vibrated out on one side. The upper panel struck his's right fourth fingertip, fracturing the bone.

EE was in the cab of stock truck, when he noticed the passenger side door was open. EE not thinking placed his left hand on the door post of the hinged side and reached with his right hand slamming the door closed on his left thumb. He received an open fracture of the tip of his left thumb.

Employee was walking across a snow-covered roadway and slipped and fell on to the frozen ground. He finished working his shift and his regular shift the next day. He did not report to work on 12/17/04, resulting in a lost time. He suffered a back sprain due to this accident.

EE was installing a screen frame in the wet mill using an open end wrench. The wrench slipped off of the bolt head and EE's hand struck the edge of the frame creating a laceration wound that required eight stitches to close.

victim was stacking 50lb. bags of sand on a pallet. victim reached out for a bag that was falling on to stacking table and the bag hit the tip of his ring finger on his left hand and bent it backwards. victim was taken to emergency room where it was x-rayed and it was determined that he had a closed fracture of the finger.

A NON EMPLOYEE TRUCK DRIVER WAS ATTEMPTING TO HELP THE TOWMOTOR OPERATOR WHO WAS STUCK ON THE ICE ON THE LOADING DOCK. HE WAS PUSHING ON THE LOAD CAGE AS THE TOWMOTOR DRIVER ROCKED THE TOWMOTO R BACK AND FORTH. AS THE TOWMOTOR ROCKED FORWARD HIS RIGHT ARM BECAME PINCHED BETWEEN THE CAGE AND THE LAST PALLET OF BAGGED SAND, BREAKING HIS RIGHT WRIST.
