Trucker working for Vitell: trucker was trimming (knocking stones off side truck) his load slipped and fell 12 feet landing on his right side hitting his arm, leg and head on the ground which had stones there. A trucker on the scale saw him on the ground and went inside for help.

Showing all 19While placing a large glass panel into the dumpster, it slipped from his hand hitting the dumpster and breaking. A piece of glass sliced the arm an inch above the wrist on the underside of the arm.
He heat up a piece steel to bend. He repaired a flange on the chute for plant, was approx. 20' of the ground. 1 leg was on the chute, other leg on the main frame structure holding up the plnat. He use his r. hand for balance. holding the frame with his r hand to steady himself he hold torch in his L hand, accident pulled torch across his r. hand wrist.
Hydraulic cylinder on the Kamasu PC-300 was being replaced. EE was on the lader pushing pin thru. Bucket was holding cylinder up. Clay was packed on the back of the bucket. Small amount of clay broke down tram bucket and fell on his head. The ladder went one way and he the other way. He and other the dirt and his left leg buckled under him.
Area not used in Plant B. EE climbed onto the cat walk circumventing the blocked off stairway. EE of his own acord decided to clean the 3/8 flat converyor cat walk in this blocked off area. He stepped onto an area with no grating. While falling he grabbed the safety railing on the cat walk, pulling the muscles in his left shoulder.
EE was taking fan guard off a 988 loader using a 3/8" rachet to unbolt it. The rachet became unlocked, right hand banged into a metal post on the loader. Over time the knuckle became swollen. Pain in hand did not go away.
EE was servicing the haul truck. He was climbing the ladder. He hit the inside bone next to the knee cap on the corner edge of the step on the ladder. No lacerations occurred at the time of the accident. The injury point was sore. EE called in accident on 5/3/06, then sought medical treatment, because knee was stiffening up and not getting better.
All day employees were rebuilding crusher of B plant. Using crane to lower chute back in place between the bypass chute & chute, employee on the crusher itself by the bypass chute was guiding the chute piece into place when it became pinched between chutes. When he released the chute it swung down smashing his right hand between chute & bypass chute (x-ray; no fracture).
EMPLOYEE WAS WORKING ON 8 X 20 H&R SCREEN. THE METAL SCREEN HAD SOME JAGGED RIPS IN IT, EE'S WRENCH SLIPPED OFF THE NUT WHICH CAUSED HIM TO HIT THE RIP IN THE SCREEN. HE HAD A 2"LACERATION ON HIS FOREARM, LACERATION WAS DEEP. EE DID NOT WANT MEDICAL TREATMENT AT THE TIME, FIRST AID WAS RENDERED. LATE AFTERNOON HE WENT TO THE HOSPITAL & 2 STITCHES WERE NEEDED.
Was stepping off WA700 Loader. Employee was coming down the ladder, he stepped on a rock & twisted his left ankle.
PRIMARY CRUSHER CHUTE PLUGGED UP. RAN DOWNSTAIRS TO UNPLUG. IT WAS AN EMERGENCY SINCE NEW TRUCKS HOLD MORE STONE THAN CRUSHER CAN HANDLE. WHILE UNPLUGGING A ROCK FLEW INTO RIGHT EYE. BLOOD SPO T WAS IN EYE. WENT TO EYE DOCTOR TO GET CHECKED ON.
WAS DRAINING OIL OUT OF OIL TANK ON PRIMARY CRUSHER. DRAIN LINE WAS PLUGGED, USING STICK TO UNPLUG CLOG CAME LOOSE AND FELL INTO OIL REFUSE BUCKET AND SPLASHED INTO RIGHT EYE.
USING FOOT ON WRENCH TO GET LEVERAGE TO FIX A MACHINE, AFTERWARD KNEE HURT SLIGHTLY. AS DAY WENT ON IT STARTED TO HURT MORE.
AT APPROX 8:45 AM INDIVIDUAL WAS TRIMMING CONVEYOR BELTING FOR FLASHING WHEN THE UTILITY KNIFE HE WAS USING SLIPPED FROM THE BELT, CUT INTO HIS LEFT ARM CREATING A CUT APPROX 18-20 MM IN LENGT H BY 10-12 MM IN DEPTH.
CUTTING STEEL TANK, SLIPPED AND FELL. RECEIVED 3" ABRASION ON HIS RIGHT LOWER LEG. ACCIDENT WAS 6/15 BUT REPORTED ON 6/20. FIRST DAY OUT WAS 6/21.
HAND HIT TUNNEL BELT AND INJURED TENDONS.
WHILE LOCKING UP TRUCK FOR THE EVENING, HE SLIPPED & FELL ABOUT 4-15 FT TO THE GROUND. ANKLE FRACTURE TO SEE ANOTHER DR FOR BETTER RESULTS
CLEANING CRUSHER FROM UNDERNEATH, A STONE FELL &HIT HIS EYE CAUSING A SLIGHT CUT TO HIS CORNEA. A STONE CHIP WAS ALSO REMOVED FROM HIS EYE.
