While loading the forks for the skid steer the employee was moving it into position by hand. The fork shifted and employee lost their grip and the tip of the fork smashed their right index finger.

Employee was attempting to rotate a conveyor head pully and the pipe wrench handle broke resulting in a sore wrist. The injury was treated with first aid only measures until a medical doctor placed them on light duty on 11/15/2023 and notified us on 11/21/2023.
