EE was on ladder fixing hydraulic lines on the Excavator and fell on the ground
Lowering a Stacker Conveyor while putting pin in to hold in place EE tried to hold up conveyor with EE's knee and slipped and pined EE's ankle under the frame then rolled EE's ankle. that resulting fracturing EE's ankle.
While checking Feeder Hopper, fell back and cut arm on expanded metal.
EE was trying to pry a rock out of jaw unit and smashed EE's finger with pry bar and the rock.
Miner was assisting in the removal of a boulder from a jaw crusher. While working, another smaller rock near the work was dislodged and fell, smashing the miner's finger between this rock and the boulder they were working to remove. The impact caused a laceration on the right middle finger, requiring stitches.
Miner was replacing V-belts (drive belts) for a Rolls crusher drive system. While performing this operation the miner's finger became stuck in between the drive sheave and the belt while rotating the sheave to put the belt in place. This lacerated the miner's right index finger. Miner's laceration was stitched at a local ER.
Worker was helping to load a welding skid on a trailer. When the skid was lifted the load started to swing. The worker reached out Right hand to stabilize the load. The little finger of the hand was pinched between the skid and a truck scale already loaded on the trailer. This smashed the finger between the two objects and broke the skin on the little finger. Required stitches.
Setting a conveyor belt in place. The worker was placing blocking under the conveyor support when the load shifted downward, smashing the worker's Left thumb between the block and the conveyor leg. This cut the tip of the thumb and required stitches to close the wound.
Worker was changing a v belt when EE's RT little finger was pinched between the belt and the sheave. The worker did not immediately report the incident thinking there was no injury. A later visit to a DR. revealed the finger is fractured. Worker reported injury July 9th.
Employees were cleaning the receiving lip of a screen box. There was wet clay material on the punch plate screens within the screen box. While standing on this screen the employee slipped on the wet clay, fell backwards, and hit their head, causing a gash in the scalp. The gash was accompanied by a raised bump and swelling. The laceration was closed with 2 staples.
The miner was preparing to set crane cables to lift a conveyor. The conveyor was lying flat on the bed of a trailer. The miner attempted to set the crane hooks by standing on the trailer tire. The miner lost grip/footing and turned to jump to the ground. When landing, miner's right foot was supinated, causing a tibia fracture.
ee bent over to pick up a 50 pound bag of shot powder & felt lower back pain. The pain continued to gradually get worse over the next few days.
EE was trimming conveyor belt using a utility knife. The belt cut easier than EE expected, resulting in the knife travelling past the conveyor belt and making contact with the EE's leg. The contact resulted in a laceration requiring sutures.
During Teardown of the loadout bunker one of the legs got hung up. They were using a pry bar to release the tension, the bar slid off the leg and it slid down on the pedestal. EE had put hand on the pedestal to move it as the bar slid off crushing EE's 4 fingers.
the employee was sitting on the load out bunker belt preparing to hang the remote. when EE pushed the button and sending EE off the belt. employee fell (Flew) 18 feet to ground. broken wrist, nose and chipped 2 teeth.
EE became unresponsive while operating the 982 loader. Called 911 we went to hospital and was found to have had a brain aneurysm.
EE's wrist, hand and shoulder hurts. Does not know how it got that way. "been bothering EE for some time"
Rolled hand into V-Belt Pulley, Fractured Knuckle.
Employee was going to strap rock that was stuck in cone, the plant man ran the cone cleanout backwards causing the rock to fracture. Part of the rock struck employee in the face.
Laborer kneeling & stemming blastholes set hand shovel down behind self, truck moved forward with spotter direction, approx 5mph. Passenger front tire caught shovel-end; shovel handle kicked up and knocked laborer to the ground, legs extended in tire path. Tire ran over legs. No fractures or lacerations were sustained. Employee was released from the hospital same day.
Employee was Standing with their back to a burn barrel to warm up and caught their coveralls and coat on fire. Resulting with 2nd and 3rd degree burns on their butt and upper legs.
On 11/14/18, Miner was digging a hole by hand on a jobsite. When ee stood up, ee felt a head rush that followed with a persisting headache. On 11/21/18 ee reported headache to the company, sought medical attention, and was subsequently diagnosed with a subdural hematoma. Treatment notes state condition should heal on its own with modified activity for 4 weeks.
Employee was attempting to clean out Jaw conveyor tail pulley when another employee bumped the power, pulling employee's arm into the tail pulley.
Rock was to hot on the burner went back to take the pan off the burner and some of the rock started popping or exploding causing fragments of rock and very small fragments of metal from the pan to become airborne, one of the fragments of rusted metal went into right eye.
Employee was closing the rolls when he walked outside of the designated catwalk and stepped on to a belt guard that was slick causing him to fall 12 feet to the ground. He broke his left leg and his nose when he hit the ground.
Haul truck operator backed of off stockpile approx. 25ft. landed upside down. operator was not wearing seatbelt. Berm was not in place where accident occurred. Miner has head and chest trauma.
Co-worker was in the process of shutting down the generators, to do maintenance to the power cables. At that time employee started to unguard the power cables on the switch van out of the vision of co-worker. He refused to have the power checked by two other co-workers. Moments later he was electrocuted.
EE WAS FEEDING CRUSHER WITH 988F AND WAS MOVING TO THE QUARRY FACE AND RAN THE BUCKET INTO THE GROUND STOPPING THE LOADER ABRUPTLY, CAUSING EE TO FALL OUT THE FRONT WINDOW AND LANDING ON THE B UCKET.
THERE WERE NO CONDITIONS AT THE WORK SITE WHICH CONTRIBUTED TO THE EE'S DEATH. EE DIED OF A HEART ATTACK WHILE OFF DUTY AND IN HIS OWN VEHICLE.
