While lifting angle iron with a telehandler, a fork caught on a welded piece of metal on the side of the scale platform. When the weld broke, the fork sprang upward, jolting the angle iron upward, which struck the mechanic's jaw. First aid was applied, but on 5/21/25 EE reported ongoing pain. On 5/27/25, a CT scan revealed a closed fracture of the right maxilla.

Showing all 50While cleaning in a tight area with shovels, another employee slipped in the mud and impacted the employee in the mouth with the wood handle of a shovel.
An employee was assisting in loading a roll of a conveyor belt into the back of a service truck. The employee was standing in the back of the service truck. As the roll was coming off the forks of the telehandler, it gained more momentum than planned and the roll pushed the employee's lower legs against the air tank reservoir behind them.
Hernia due to overexertion form improper lifting
An employee was standing on a step and the v belt guard was facing the screen. EE turned to get a new screen rail and stepped off the v belt guard. EE grabbed the shaker with EE's right arm as it was still over the top of the shaker and caught self preventing falling to the ground.
Miner finished welding under the primary jaw crusher while wearing a welding/grinding hood. They switched to a face shield and used a wire wheel to clean the welds. They removed the shield, put on their safety glasses, and began moving the tools out from under the jaw crusher. Wind moved some of the dust as they climbed out. They then felt something in their left eye.
The pigtail of a crane swung and caught the employee. Knocked off forehead and caused a laceration on EE's forehead.
A piece of metal from grinding fell under the safety glasses and got into the eye. Significant delay in reporting the injury.
While scooping dirt, the fire extinguisher at the employee's feet discharged spraying employee's left side and face. EE drove the skid steer to the air compressor and blew the powder off person and the cab of the skid steer and let the supervisor know. EE was still experiencing shortness of breath days later so EE got an inhaler to assist in reducing symptoms.
Miners were replacing a screen bearing at SR07 at the Fountain Pit Plant. They had to take tools and new parts up and down the catwalk area which is at an incline and work on the bearing replacement. An employee reported the next morning when EE got to work that when EE got home the night before, EE started feeling soreness around the lower back and right hip area.
Employee was taking off a guard from a conveyor when the screen guard slipped from employee's hand and pinched the tip of middle finger on right hand causing a need for 5 stitches
Employee was descending a set of metal stairs from a screen deck when they tripped causing them to land awkwardly on the next lower step. The sudden jarring caused discomfort in their lower back.
A rock hit the employee's hard hat and then his right shoulder. Employee was taken to the hospital treated and released with no major injuries.
Employee was attempting to close the sliding door of dumpster that was stuck. He asserted force in attempt to shut the door, it closed rapidly catching his left index finger between door and frame of dumpster.
Miner was on his knees shoveling around conveyor for a period of time. He stood up and turned his body to leave the area. His left knee popped.
A rock came off of a conveyor and hit employee on left arm. Arm was bruised.
Two employees were moving rock from creek bed. As they picked up a rock, another rock slid down a little catching his little finger in between two rocks.
Employee was doing pre-shift inspection - it was still dark. He stepped on a rock that was lying on the catwalk & twisted his ankle.
EE was using a utility knife to cut rubber belting. The blade went through easier than expected and the blade struck him in his right thigh. 3 stitches were required to suture the wound.
EE was rolling a screen in position with left hand and using a small hand grinder on the screen. Grinder caught on screen, kicked the grinder back striking ee on left thumb.
Employee was using a bladder pump to remove water from a bore hole when bladder ruptured forcing water & small rocks up the hole. The force of the water pushed up his safety glasses allowing material in his eyes.
Employee attempted to put a fuse into fuse box which had power, causing electrical arc which burned his arms.
EE missed last step coming down from ladder and fell 2' to the ground. His ribs landed on a frozen dirt clod, causing rib contusion.
Pinched finger between rock & piece of steel.
Operator of shovel was loading large boulder onto rear end of haul truck(60 ton haul truck). The boulder slipped out of bucket and dropped into bed of truck. The bed raised up and then came back down with force causing whiplash type injury to haul truck driver.
Employee was shoveling at plant, he felt pain in his elbow which lasted about 5 minutes. After pain stopped, elbow filled with fluid.
Employee had mud on his boots. As he dismounted his loader, his foot slipped off the bottom rung. He fell to the ground spraining his right ankle.
EE claimed of pain in her R. wrist. An evaluation was done to determine the cause of pain, she has carpel tunnel in her R. wrist.
Employee was feeling ill. He got into his personal vehicle and drove himself to a medical facility. While he was at the medical facility he suffered a heart attack. He was transported to a hospital where a stent was placed. This is a one person operation. EE did not alert anyone of the problem. He left the property to go to the clinic.
Loader operator climbed down from cab of loader, he turned around, once he got to the ground & started to walk away from the loader he stepped into a hole & fell forward. His left knee struck a 5"rock that way laying on the ground.
EE was using a torch to cut a weld. He turned his head and accidently pulled the torch across his hand.
EE STEPPED ON THE EDGE OF A ROCK LAYING ON THE GROUND, LOST HER BALANCE AND FELL, INJURING HER RIGHT SHOULDER.
EE STEPPED UP ON A BOX DURING A SNOW STORM AND SLIPPED OFF OF IT DUE TO WEATHER CONDITION AND FELL BACKWARD TO GROUND-HE WAS 1 FOOT OFF OF THE GROUND.
EMPLOYEE WAS LIFTING A DRUM FROM AN AGGREGATE STACKER WHILE ANOTHER EMPLOYEE WAS LINING UP THE HOLES TO INSERT A BOLT. EMPLOYEE WAS ON HIS KNEES WHILE LIFTING. HE FELT PAIN IN HIS KNEE OVER TH E WEEKEND AND SWELLING HAD OCCURRED.
EMPLOYEE WAS CLIMBING UP LADDER TO GET IN LOADER CAB & HIS FOOT SLIPPED OFF THE LADDER RUNG. WHEN HE SLIPPED, THE WEIGHT OF HIS BODY PUT A STRAIN ON HIS RIGHT SHOULDER.
EMPLOYEE WAS HELPING PLACE 80 LB LOCKS AROUND A FOUNDATION FOR A TRAILER USED AS AN OFFICE. HE MUST OF STRAINED HIS LOWER BACK. DID NOT FEEL PAIN UNTIL WEEKEND - REPORTED INCIDENT ON 6-30-03.
EMPLOYEE SLIPPED GETTING DOWN FROM LOADER & FELL BACKWARDS, HITTING HIS HEAD ON THE TIRE OF ANOTHER PARKER LOADER.
EE CUTTING CONVEYOR BELT WITH UTILITY KNIFE. KNIFE SLIPPED & CUT INTO LEFT PALM OF HAND. 7 STITCHES REQUIRED.
EE WAS HELPING TO CHANGE SCREENS WHEN HIS HARD HAT FELL OFF AND DOWN TO THE BOTTOM SCREEN. HE CRAWLED OUT TO GET THE HARD HAT & BUMPED HIS HEAD ON A BRACE.
EE WAS LIFTING UP ON A CONVEYOR BELT WHILE ANOTHER EE WAS CHANGING OUT A RETURN ROLLER.
EE WAS HELPING TO CHANGE SCREENS AND WAS CRAWLING AROUND UNDER THE TOP SCREEN WHEN HE FELT HIS KNEE POP. EE PLACED ON RESTRICTED DUTY.
EE SLIPPED ON ICY AREA WALKING TO HIS LOADER. THAWED CONDITIONS THE PREVIOUS DAY RE-FROZE OVERNIGHT. AREA WAS SEMI-DARK THAT EARLY IN THE AM BEHIND THE MOUNTAIN WHERE THE SUN HADN'T SHONE YET. WHEN EE SLIPPED HIS RIGHT LEG WENT FORWARD CAUSING A SPLIT. HE WENT DOWN ON THE LEFT LEG AND FRACTURED HIS LEFT LEG FROM IMPACT AT THE KNEE AND WEIGHT OF BODY.
EE WAS HELPING TO RAISE A SUBMERSIBLE PUMP WITH A CHAIN WRAPPED AROUND THE FRAME. THE CHAIN SLIPPED PINCHING FINGER BETWEEN FRAME AND CHAIN.
EMPLOYEE SLIPPED ON ICE WHILE WALKING AROUND IN PLANT AREA. HE LANDED ON HIS BACK.
EMPLOYEE WAS SPLITTING A ROCK WHEN A PIECE OF THE ROCK FLEW BACK AT HIM AND STRUCK HIM ON THE LIP.
EE FELT PAIN IN WRIST AS HE WAS PERFORMING MAINTENANCE WORK ON PLANT.
EE IS A STONE CUTTER. IN SPLITTING A ROCK, 1/2 OF PIECE FELL ON EMPLOYEE'S LEG, INJURING THE KNEE.
EE WAS HELPING TO CHANGE OUT SCREENS WHEN THE TOP SCREEN SLIPPED, CAUSING HIS LEFT THUMB TO BE PINCHED.. THE DR. PLACED HIM ON RESTRICTED DUTY UNTIL THE NEXT VISIT.
THE EE WAS STRUCK IN THE CHEST BY A BAR WHILE WORKING ON AN OVERSIZED CONVEYOR.
EMPLOYEE WAS STEPPING DONW FROM CONVEYER WHICH HE WAS WORKING ON, AND STEPPED ON A LOG CAUSING HIM TO STRAIN HIS LEFT LEG.
