While trying to dislodge the bulk loading spout with a pry bar, the pry bar hit the teammate under the chin causing damage to four of the teammates teeth on the right side of EE's mouth.

Using pneumatic jackhammer to tear out refractory lining inside the cylinder wall of kiln. Worked up wall to about shoulder height when jackhammer vibrated backward. Employee's right hand was caught in the handle. Diagnosed as soft tissue strain. Returned to work the next day before traveling to follow up with home doctor for possible ortho referral.

On 6/22/21 at 10:00am EE was restacking pallets alone on the east wall of packaging of MD-09 due to poor stitching resulting in leaking bags. EE made a bending twisting motion while moving a bag from one pallet to another. EE felt a sharp pain in middle right section of back.

while walking down the icy stairs, employee fell and tried to catch themselves on the hand rail and felt a pop in shoulder. EE was medically evaluated at the time and was released to return to work they continued to feel pain in shoulder and had a second evaluation which determined they had to have surgery to repair the damage.

Was opening lid on pneumatic tanker when ee pulled up on lid hand slipped. When ee was climbing back up the ladder, ee noticed finger was bent a little and was not completely straight. The doctor is stating that ee has acute mallet- finger defect (ruptured extensor tendon) to distal interphalangeal joint of RT 4th (ring) finger.

EE was cutting a band off of pallets using a utility knife to cut the banding material. EE had the band partially cut and the knife struck EE's left wrist cutting it. EE required 3 stitches and antibiotics.

While chocking a dust truck; employee bent over to put chock in place and dropped chock into pile of dust. There was a hole in the chock; and the dust blew through this hole and went into teammates eyes. Teammate did have on safety glasses. Teammate went to clinic and had eyes washed out and checked for scratches. Prescription given for eyes.

Employee walking from truck shop to office, slipped and fell. Employee stated he slipped and fell in mud, behind trucks in front of his office. Oil-rig is disputing this claim due to no witnesses, no evidence in area that a fall occurred, and the employee showed no signs of soiled clothing, no mud on clothes.

Employee was walking by fan carrying some grating to make a walkway in the tower area. It was muddy and the way he had to position the metal to get through the walkway he slipped and hit the edge of the guard with his right forearm.

Employee was coming down off C22 ladder and there was snow and ice at the foot of ladder. When he stepped on the ground he slipped and twisted his knee. Employee had not complained about any pain until February. He was sent to the doctor on 2/17/2010. He had not missed any work until 4/15/2010 when he had surgery.

Packaging palletizer #2 feed conveyor was coming apart at the splice. Employee asked forklift operator to get the lift cage with the forklift to assist him in the repair. While the employee was in cage he leaned forward and the cage and employee fell from forklift. The fall was from 4 feet. The employee sustained a head cut and bruised elbows.

Employee was tugging on a rope, trying to get it unstuck. A part of the rope was braided & stiff. The braided, stiff part of the rope went across employees face knocking off his safety glasses. employee has an l shaped abrasion on his left corneal. Oil-dri was notified on 1/4/05 that the ee was given prescription medication, which would qualify this incident as an msha reportable.

ON 6-27-01, EE WAS WELDING A LID FOR A BLOWER FILTER, WHEN A PIECE OF SLAG CAME UNDERNEATH THE WELDING HOOD, ENTERING HIS RIGHT EYE. ON 6-28-01 HE REPORTED THAT HE HAD SOMETHING IN HIS EYE. HE WENT TO THE E.R. AND HAD SOME OF THE OBJECT REMOVED. ON 6-29-01 MARION EYE CENTER REMOVED THE REST OF THE FOREIGN BODY FROM HIS EYE. HE WAS OFF FOR ONE DAY.

2 EMPLOYEES WERE LIFTING AN AIR-CONDITIONER TO INSTALL IN THE PACKAGING OFFICE. THE AIR-CONDITIONER SLIPPED OUT OF EE'S HAND AND FELL ON INJURED'S HAND. THIS RESULTED IN A CUT ON INJURED'S 3RD FINGER ON HIS LEFT HAND. THE CUT REQUIRED 10 STITCHES.
