Teammate fell backwards while restacking a pallet from the conveyor

While inspecting the 20' Lilly Auger for a leaking hole teammate inadvertently placed a finger inside of the auger from the bottom panel.

While trying to dislodge the bulk loading spout with a pry bar, the pry bar hit the teammate under the chin causing damage to four of the teammates teeth on the right side of EE's mouth.

Using pneumatic jackhammer to tear out refractory lining inside the cylinder wall of kiln. Worked up wall to about shoulder height when jackhammer vibrated backward. Employee's right hand was caught in the handle. Diagnosed as soft tissue strain. Returned to work the next day before traveling to follow up with home doctor for possible ortho referral.

On 6/22/21 at 10:00am EE was restacking pallets alone on the east wall of packaging of MD-09 due to poor stitching resulting in leaking bags. EE made a bending twisting motion while moving a bag from one pallet to another. EE felt a sharp pain in middle right section of back.

While using a knife to cut a belt the knife slipped off the belt and caused a laceration to the upper area of the right knee.

while walking down the icy stairs, employee fell and tried to catch themselves on the hand rail and felt a pop in shoulder. EE was medically evaluated at the time and was released to return to work they continued to feel pain in shoulder and had a second evaluation which determined they had to have surgery to repair the damage.

While sewing bags on F-line employee stated they felt a pain in left elbow. Employee felt this pain for a month prior to seeking medical treatment.

Operator ducked to cross under a beam, EE did not duck low enough and the area of EE's forehead between EE's hard hat and safety glasses struck the beam.

Was opening lid on pneumatic tanker when ee pulled up on lid hand slipped. When ee was climbing back up the ladder, ee noticed finger was bent a little and was not completely straight. The doctor is stating that ee has acute mallet- finger defect (ruptured extensor tendon) to distal interphalangeal joint of RT 4th (ring) finger.

EE was cutting a band off of pallets using a utility knife to cut the banding material. EE had the band partially cut and the knife struck EE's left wrist cutting it. EE required 3 stitches and antibiotics.

While chocking a dust truck; employee bent over to put chock in place and dropped chock into pile of dust. There was a hole in the chock; and the dust blew through this hole and went into teammates eyes. Teammate did have on safety glasses. Teammate went to clinic and had eyes washed out and checked for scratches. Prescription given for eyes.

Employee was driving down highway, made a right turn and steering wheel locked up on tractor/trailer causing shoulder pain.

Employee was lifting wheel chock to put on front end loader and strained his left shoulder/neck.

Employee walking from truck shop to office, slipped and fell. Employee stated he slipped and fell in mud, behind trucks in front of his office. Oil-rig is disputing this claim due to no witnesses, no evidence in area that a fall occurred, and the employee showed no signs of soiled clothing, no mud on clothes.

Employee was walking by fan carrying some grating to make a walkway in the tower area. It was muddy and the way he had to position the metal to get through the walkway he slipped and hit the edge of the guard with his right forearm.

Employee was coming down off C22 ladder and there was snow and ice at the foot of ladder. When he stepped on the ground he slipped and twisted his knee. Employee had not complained about any pain until February. He was sent to the doctor on 2/17/2010. He had not missed any work until 4/15/2010 when he had surgery.

employee was on break and walking to employee parking lot to use cell phone. employee slipped on snow and ice. employee fell and injured shoulder and elbow.

Employee notified supervisor that he had had surgery on his arm, and that the doctor told him it was job related. The doctor said it was nerve damage from laying arm on window seal of truck while driving.

Employee was loosening a bolt on high temp elevator. While trying to loosen the bolt he suffered a sprain.

Packaging palletizer #2 feed conveyor was coming apart at the splice. Employee asked forklift operator to get the lift cage with the forklift to assist him in the repair. While the employee was in cage he leaned forward and the cage and employee fell from forklift. The fall was from 4 feet. The employee sustained a head cut and bruised elbows.

Operating a line small pain in hand after lunch went to B line about 1 hour stopped to dump bags and saw hand was swollen.

Employee was coming down the screening tower stairway and fell down the stairs. Employee hit her left knee on the stairs causing a bruise to the knee cap area.

Employee was placing bags into bag rack when bags fell through and the employee reached to retrieve the bags. The guide rod to the upper separator caught the employees middle finger on his right hand and smashed it.

Employee complained of pinky finger and ring finger on right hand were swelling. Employee stated, the constant grabbing bags and putting it onto a spout, caused the swelling.

Employee was working off table. Opening a valve with a pipe wrench and the wrench slipped off of stem causing causing him to lose balance and struck knee on I-Beam.

Employee was walking over a pile of scrap clay to check walkways and slipped.

Employee was at the mine walking from the loader to his truck and slipped on frost that was on the ground.

The bag rack had jammed and the employee was trying to get it out. The arm on the rack came down on the employees right knuckle. Right knuckle was swollen.

Employee was going out of door on MCC room and stepped into the mud where the concrete was broken and turned his left ankle.

Employee was opening the door on back of crusher. The door fell down and knocked him from the ladder causing him to hit his back on divider in hopper.

While gluing on B-line (25lb line) employee complained of pains going in and out of her back and left leg.

Employee was climbing down ladder on Griffin Baghouse. While climbing down he felt his left shoulder pop. Employee had discomfort in his shoulder during the night and went to the doctor the next day.

Employee was carrying a filter housing up a ladder. He propped the unit on the frame of the blower. Filter house slipped off the blower and caught his left middle finger between the blower and the filter housing. Employee tore his left middle fingernail and crushed the tip of his left middle finger.

EMPLOYEE WAS GRINDING, CUTTING & WELDING WHEN SOMETHING GOT IN HIS LEFT EYE. EE WAS TAKEN TO THE E.R.& METAL WAS REMOVED THAT WAS EMBEDDED IN EE'S LEFT EYE. EE WAS WEARING SAFETY GLASSES.

EE CUT RIGHT SHIN ON A FAN BLADE WHILE ASSISTING IN REMOVAL OF FAN. EE SAID HE DID NOT NEED MEDICAL ATTENTION. ON 02/02/05 AND 02/11/05, EE WENT TO CLINIC FOR AN INFECTION TO THE WOUND. ON 2/14/05 THE DOCTOR LANCED THE WOUND. EE RETURNED TO WORK ON 2/16/05

Employee was tugging on a rope, trying to get it unstuck. A part of the rope was braided & stiff. The braided, stiff part of the rope went across employees face knocking off his safety glasses. employee has an l shaped abrasion on his left corneal. Oil-dri was notified on 1/4/05 that the ee was given prescription medication, which would qualify this incident as an msha reportable.

EMPLOYEE SLIPPED ON STEP AND TWISTED LEFT FOOT. EMPLOYEE WAS TREATED AT THE HOSPITAL AND WAS DIAGNOSED AS HAVING HIS LEFT ANKLE BROKEN IN TWO PLACES.

EE WAS FUELING TANK ON SERVICE TRUCK. HE WAS RUSHING BECAUSED HE NEEDED TO USE THE BATHROOM. EE WAS DRAGGING THE FUEL HOSE AROUND THE TRUCK AND RAN HIS HAND INTO THE MIRROR ON DRIVERS' SIDE. M IRROR HAS SHARP EDGES. EE WAS TAKEN TO THE HOSPITAL AND RECEIVED STITCHES IN HIS RIGHT INDEX FINGER.

EMPLOYEE WAS CARRYING SAMPLES FROM PACKAGING LINE TO LAB FOR TESTING. EE SLIPPED WHILE CARRYING THESE SAMPLES. THE EMPLOYEE COMPLAINED OF PAIN IN THE GROIN AREA. THE EMPLOYEE WAS DIAGNOSED WITH A L INGUINAL HERNIA.

EE WAS WASHING TAIL LIGHTS ON TRAILER, THE LIGHT WAS DAMAGED & EE CUT HIS LEFT PINKY FINGER. FIRST AID WAS PERFORMED & THE EE WENT TO THE HOSPITAL & RECEIVED SUTURES IN THE FINGER.

EE WAS CHANGING LINE OVER BY MOVING CONVEYOR BELT OVER TO FLIP BAGS OVER ON RT. SIDE UP, WHEN HEFELT A SHARP PAIN IN HIS LOWER & UPPER BACK. EE WAS TAKEN TO DR. OFFICE AND PLACED ON LIGHT DUTY FOR BACK STRAIN.

EE SUFFERED SMASHED LEFT HAND & FINGER CUT (MIDDLE/LEFT) WHILE SERVICING SHAKER VIBRATOR EQUIPMENT.

EE WAS ON THE PACKAGING B-LINE, MOVING THE SEWING HEAD WHILE THE MACHINE WAS STILL RUNNING. THISRESULTED IN HIS RIGHT PINKY BEING FULLED IN THE BELT DRIVE. THE FINGERTIP WAS CUT OFF. HE WAS TA KEN TO THE HOSPITAL WHERE HE RECEIVED STITCHES. THE EMPLOYEE WAS RELEASED TO RETURN TO WORK.

EE WAS DISMANTLING SCAFFOLD, 4 ABOVE A PIER. EE STEPPED OUTSIDE HAND RAIL ON A PLANK THAT WAS NOT SAFETY WIRED-PLANK TIPPED & EE FELL DOWN

ON 6-27-01, EE WAS WELDING A LID FOR A BLOWER FILTER, WHEN A PIECE OF SLAG CAME UNDERNEATH THE WELDING HOOD, ENTERING HIS RIGHT EYE. ON 6-28-01 HE REPORTED THAT HE HAD SOMETHING IN HIS EYE. HE WENT TO THE E.R. AND HAD SOME OF THE OBJECT REMOVED. ON 6-29-01 MARION EYE CENTER REMOVED THE REST OF THE FOREIGN BODY FROM HIS EYE. HE WAS OFF FOR ONE DAY.

2 EMPLOYEES WERE LIFTING AN AIR-CONDITIONER TO INSTALL IN THE PACKAGING OFFICE. THE AIR-CONDITIONER SLIPPED OUT OF EE'S HAND AND FELL ON INJURED'S HAND. THIS RESULTED IN A CUT ON INJURED'S 3RD FINGER ON HIS LEFT HAND. THE CUT REQUIRED 10 STITCHES.

EE WALKED OUTSIDE TO GET A TRUCK DRIVER WHEN SOMETHING FLEW INTO HIS EYE. HE TRIED TO FLUSH THE OBJECT OUT BUT COULD NOT. EE WENT TO THE HOSPITAL AND WAS GIVEN 2 DAYS OFF. ON 4/19/01, THE SUPE RVISOR TURNED IN THE DR'S EXCUSE, HE STATED THE INJURY WAS NOT WORK RELATED. 4-23-01 EE SAID HE NEEDED A STATEMENT FROM WORK REGARDING HIS LEFT EYE INJURY SO HE COULD GO TO THE EYE SPECIALIST

EE WAS LIFTING A CHEP PALLET OVER HIS HEAD TO LOAD IN THE STACKING MACHINE EE FELT A SHARP PAIN IN HIS LOWER BACK. EE WAS TAKEN TO COMMUNITY HEALTH IN CAIRO ILL, AND WAS GIVEN MEDICATION FOR BACK STRAIN. EE WAS ALSO PUTON 3-2 DAYS LIGHT DUTY
