The employee was moving a 55 gallon drum while holding the barrel at an angle. He tried to push an object out of the way with his left foot. This caused a pull or tear in the groin area.

Miner adjusting a ball mill liner in the mill, when the liner swung to the left, pinched left index finger between the liner and the diaphram which caused a fracture. Miner was placed on light duty on 1/13/07 but elected to decline the modified duty postion.
Contractor suffered a broken left leg and right heel when he inadvertently stepped back off mill pier while installation of raw mill bearing housing was underway. Employee fell approximately 15'5". Employee had removed his fall protection to gather parts on the ground below and had failed to put it back on.
Miner removing liner in the ball mill, when it slid down and landed on his right foot behind the steel toe. The miner was informed on 1/3/07 that there was a fracture of the right 5th metatarsal. Miner was placed on restricted duty.
Miner initially reported on 12/26/06 that slag went through his left sleve on 12/23/2006 & his elbow was swollen. Miner stated that the alledged injury might have happened at a different operation. Miner was taken to clinic where he was diagnosed with cellulitus. He was placed on restricted duty from 12/26 thru 1/1/07.
A co-worker was pulling a drive chain back into position when the chain slipped out of his hands striking the injured worker in the right elbow.

Employee was removing liners in the mill with a sledge hammer when a piece of hammer head chipped off hitting employee in the forehead. Employee received stitches.
Employee was performing an electrial inspection/repair when he struck his right elbow on the edge of a hydraulic panel.
