During repair to a water pipe located under concrete. A jack hammer was used & connected to the welder/generator for power. The generator was turned off. When they went to use jack hammer again the generator would not start. A forklift and jumper cables were used to crank the welding machine. EE went to remove the jumper cables & EE's hand got caught in the fan.

EE was helping replace a vent fan motor. The motor was already on the catwalk and EE climbed the ladder so they could hand EE the motor. Before they could hand the motor to EE, EE slipped and fell backwards and over the railing, approx 26ft fall & landing at an angle. EE's left ankle hit first and then EE's head.

Employee was chasing an air leak at the dryer and when they went to adjust the couplings in the lines.. Upon tightening the PT it blew open and insulation particles got in their throat and ears.
Employee bagging on production line reported lower back pain. Told supervisor EE was ok. Reported to Supervisor a 2nd time that EE slipped. Co-worker reported that the employee had another Operator crack EE's back prior to the reported incident as well. Employee was treated at Hospital same day and issued a DR. excuse to return to work in 2 days without any restrictions.

Employee reported to supervisor that they did not feel good and felt very weak. EMS was called in and they did a basic check for stroke symptoms. EMS did not diagnose at that point, but they did take them to hospital where they were admitted for observation.

Employee did not realize the mill had not been flowing and the media had heated up the material in the attritor due to friction, EE then removed a hose without closing the valve which allowed the hot material to escape which caused burns to both hands.

Employee became dehydrated, and heat exhausted. EE did not drink enough fluids during EE's shift and EE's legs began to cramp. EE was taken to air conditioned area to cool off and given cool towels, ice packs, water and gatorade.

Employee was walking over a small pile of rocks and some slid out from under employee's foot causing it to bend sideways and pop. Ankle was x-rayed and did not show any broken bones. It was wrapped, employee was given crutches and medication for pain and swelling.

Employee was working on 50" mill conveyor. Afterwards EE was trying to use a paint spray when it suddenly discharged paint into EE's face. EE wiped their eyes and face with a dirty cloth and then is when EE thinks EE got something in the eye.

EE was attempting to turn off the blower, when EE turned to do so, the area was wet from the rain, EE slipped. EE tried to catch themself and their arm hit the pipe coupling bolts causing a laceration. EE was sent to local clinic and the cut was stitched, EE was given antibiotics and a tetanus shot.
Employee strained left forearm while attempting to life 6x6 tubing onto forklift. Employee is working but is on light duty - unable to lift over 20lbs.

Employee was replacing the bad bearings on the attritor mill shaft. While removing the plate, the shaft moved. The employee's left arm and wrist were pinned against the cone of the attritor mill chamber (between the cone and shaft).

EE stated EE was moving a hanging scale and could not see so EE stepped up on the side of the hyster and laid hand on the boom and raised the boom and pinched fingers.
EE was bagging at the silo and a pipe wrench handle fell from above and hit EE in the head while EE was wearing hard hat.

EE WAS CLEANING TAIL PULLEY ON #1 CONVEYOR. A CHEMICAL TRUCK SPRAYING HERBICIDE ON FENCELINE SPRAYED HIM WITH KROVAR IDF HERBICIDE. EE WAS DOWN WIND FROM TRUCK. WIND CARRIED MIST ONTO EE.
