A miner was cutting a piece of metal with a torch. A piece of hot metal fell from the cut and on to the miner's jeans. The metal melted through the jeans and fell down in to the miner's slip-on boot and came to rest on the top and outside of the miner's left foot - burning their foot.

Miner was swinging a 3 pound hammer when they felt a pain in their right shoulder and elbow.

Some ice had built up due to weather on the apron of a shop bay. Employee was attempting to move a truck off the apron so the ice could be cleaned off when employee slipped and fell, injuring left wrist.

EE was driving personal vehicle on way to leave for the day. Snowfall caused the ee to become disoriented and ee struck a stockpile. EE's head struck the steering wheel causing an abrasion to ee's lip. Injury required stitches.

Employee noticed that eyes had become irritated. Mine foreman and employee found defective heater core. Employee's eye irritation had not improved after two days and medical attention was provided. Eye doctor believed eye irritation was the result of direct contact (i.e. hand rubbing) of unknown particle or contaminate to eye.

Employee was applying downward pressure on wrench and bolt extractor to remove broken bolt from loader drive yoke. The bolt extractor broke and the employee's downward force caused him to slip and hit face/lip on nearby steering cylinder.

Employee was removing a cutting edge wear liner from loader bucket. Employee was loosening nut with air-powered tool when bolt began to spin, employee used hand to push bolt up into the wear liner's bolt socket. Weight of the tool pushed the bolt from the socket, thus allowing the bolt to spin freely. The employee's hand was cut when bolt head spun.

Operator was driving a haul truck when another truck dropped a large rock on the haul road. Operator stopped and got out of truck and rolled rock off of road. Haul road was very rough after rain and operator complained he was being jarred around while driving truck.

EE WAS USING A 40 OUNCE BALLPEEN HAMMER TO FREE A STEEL WEDGE THAT HOLDS A BLOW BAR INSIDE OF A IMPACT CRUSHER. WHILE STRIKING THE WEDGE OUT, A SLIVER CHIPPED OF THE HAMMER HEAD AND WENT INTO HIS RIGHT HAND NEAR THE THUMB.

HEAVY RAINFALL SPLASHED DIRT INTO EYE, SAFETY GLASSES WERE REMOVED SO OPERATOR COULD SEE WHAT HE WAS DOING. DIRT WAS SPLASHED IN RIGHT EYE.
