EW was operating 336F#1 CAT excavator with rock breaker. EE was found unconscious by co-worker #1 at approx. 2:00PM. They called coworker #2 for assistance coworker #2 arrived at approx. 2:06PM and they got EE out of excavator. Coworkers began CPR. An ambulance was called at 2:11PM and arrived at approx 2:30PM and crew took over. Ambulance left the site with EE at approx. 3:00 PM.

Showing all 22The dipper stick portion of the hydraulic hammer assembly was cribbed on a pallet. Employee stepped on the edge of the pallet which shook the stick and hit the outside of EE's foot. Employee said was okay and continued working. When EE went home and removed the boot, EE saw the blood and laceration and went to the hospital for treatment where EE received 10 stitches.
Employee was standing on a 4' step ladder working on a grease real. EE was lining up a bolt and while EE was reaching, the ladder slipped and EE lounged forward hitting EE's face. Employee went to the Medicus Urgent Care, Dunmore where EE received (4) stitches in EE's lip and (3) stitches above EE's left eye.
Employee was heating metal with a propane torch at Primary #1 hopper. The flame went out and EE went to relight the torch. The gas built up and exploded causing burns to face, hands and chest. EE's clothing was on fire, EE laid and rolled on the ground to extinguish fire. Employee was taken to ER for medical treatment.
Employees were working on the J50 Portable Jaw Crusher. The plant was down and employee was helping the mechanics repair the radiator. Employee was standing on the hydraulic tank handing the mechanics the parts. Employee then walked onto a ledge (6-8") to get down, lost footing and fell approx. 10-11 feet.
Employee was bent over cutting a piece of conveyor belting on the screen deck and while trying to cut though the rubber, EE cut EE's right leg (calf). EE received first aid and went to the Medicus Urgent Care where EE received three stitches.
Employee was walking to weld shop and the water truck was parked next to door. He walked behind the water truck and a metal bar was sticking out. He ducked and thought he cleared the bar but he didn't and he hit his head on the bar.
Employee was using a razor knife to cut wire and his hand slipped and he cut his finger on left hand.
Employee was constructing a skid base that was elevated approximately 3' off the ground. The forklift operator was putting a beam on deck when the strap slipped causing the beam to move. Employee jumped off skid to avoid swinging beam and suffered injuries as a result of the fall.
EE was changing the cutting edge on the JD844. While holding the edge with one hand and removing the bolt with the other, the bolt spun. It cut through his glove and cut his finger. First aid was admin. and EE was taken to the hospital for care.
EE was changing cutting edges. The cutting edge were sitting on the jack and he had his hands liner the edge lining up the holes. Another EE came to help and moved the jack. The edge was wedged against the bucket. The edge released pinching ee left hand.
EE was struck in the head by a bar he was using to remove a cylinder pin. When the pin came out, the shear jaw dropped about 2 inches which caused the bar to swing and hit EE's head. EE was taken to CMC Hospital where he required 6 stitches above his right eye.
EE was climbing up onto his D8L dozer. He slipped attempting to pull himself up the first step onto the track pads to reach the second step. He fell backwards first landing on his feet, then hitting his tailbone & then his back on the ground. Weather conditions were good and temperature was approx 41 degrees. He was covered with a blanket and was transported to a local hospital.
He was manually sliding a steel plate off a wooden pallet so he could hook a clamp to it. The plate slide quick and pinched his finger against a wheel on the welding machine next to him.
The vendor was backing up and his foot slipped off the brake and hit the gas pedal causing the van to accelerate and strike the employee.
He was grinding a pulley wheel. His hand slipped and the grinder wheel struck his left hand middle finger.
Attempting to move a portable litter fence. He stopped forklift with the forks under side of the fence & exited the forklift. With forklift running, he went between the cab & lifting mechanism of the forks to chain the fence to the fork cage. He stepped backwards against the control lever & mechanism moved backward, pinning him between cage & the roof of the cab.
HE WAS WINDING THE FUEL HOSE AND IT WAS TWISTED. WHEN HE HIT THE BUTTON THE NOZZLE POPPED OFF AND HIT HIM IN THE RIGHT SIDE OF THE FACE.
EMPLOYEE WAS CHIPPING SLAG OFF WELDING AND HE STRUCK HIS FINGER.
EE WAS REPLACING THE HEAD OF A 12 LB SLEDGE HAMMER WHEN EE STRUCK THE PINKY FINGER ON RIGHT HAND
EE WAS CLEANING MUD FROM THE MODIFIED STACKER OF PRIMARY #1. THE EQUIPMENT WAS MUDDY FROM THE PREVIOUS NIGHT'S STORM. EE SLIPPED ON WET MUD ON PLATFORM AND TRIED TO JUMP OFF (4') TO PREVENT A FALL. WHEN EE LANDED ON HIS FEET HE ROLLED OVER ON HIS RIGHT ANKLE, CAUSING PAIN.
Employee was moving in and around a litter fence and tripped over a section of the fence. he fell backwards onto the piping. An ambulance was called where he was taken to the hospital and treated for three broken ribs and a collapsed lung.
