Employee slipped on ice.
A miner entered the air separator to clear a blockage. The miner became engulfed in the material.
Miner did not report to work on 06/12/2019 and was contacted by their Supervisor and questioned about their absence. Miner told the Supervisor their back was hurting from driving a rock tuck.
A rock bounced over the top of the screen and landed on the middle of the upper back. This resulted in some muscle spasms and tightness in the back.
