During roof prep work, contract employee stepped through a roof hole while moving a piece of plywood that was covering the hole. He was secured with fall protection but suffered leg strain when he leaned back to restore his balance. He was placed on modified duty for three days.

Driller was lifting a 5 gallon can of compressor oil to put into reservoir of drill during maintenance, he felt a pain in his shoulder area, went to hospital and they said it was a strain to tendon.

Employee was exiting control booth and slipped on the dry, 3 step stairway while descending. He caught himself before falling and complained of pain in his knee. Was seen and determined to have a slight strain. Employee was sent back to work with no restrictions. This became a reportable incident on 1/3/13 when treatment went from first aid to medical treatment by definition.

Emergency Medical Services was called to assist an employee who was not feeling well and had momentarily lost consciousness. EE was transported to the hospital and later diagnosed with heart valve impairment.

EE was attempting to move a fallen box back onto a pallet. As he started picking up the box, a pump inside the box moved, causing his hand to become pinched between the box and the floor. When he pulled back his hand and removed his glove, he realized he had a laceration on two fingers. Treatment was first aid and stitches.

EE was using a sledge hammer to hit the side of a chute in an attempt to dislodge the material in the chute. As he swung the hammer, his right index finger made contact with the door bracket, causing a laceration that required stitches.

EE was walking up out of a shallow bin area when his foot slipped and he grabbed onto the wall to keep from slipping further. He sprained his shoulder during this action, resulting in restricted duty work.

Employee was lifting the lid for # 4 silo in order to measure how much was in the taank. The lid was hard to open initially. Employee was bent/twisting his body and yanked on the lid when it did not come up the first time. Employee felt a pull in the lower part of his back. Employee was bending at his waist & not using his legs & not using good body opsition to lift the lid.

While removing a taper lock with a chisel, ee felt a slight irritation in the inside corner of his left eye. As time went on the irritation worsened. Looking in the mirror ee could see a little black dot in the eye.

While clearing some low brush, ee brought his arm down on a piece of metal causing a puncture. This injury was initially treated as first aid only but on 7/2/2007 he was scheduled for surgery for possible repair of tendon.

On 1/30/07 employee alleged he hurt his lower back, stating he believed it happened on 1/24/2007. His job duties included the following: Sweeping, Using a forklift, Making up boxes, Moving product from conveyor belt to the formed box, Closing up the completed package, Set up pallets for next load.

Two EEs were moving a tote container with approximately 20-50 lbs of product when EE experienced very sharp pain in lower back. EE's were using push-pull method when injured EE twistd to see the direction and area into which they were heading. When EE twisted it appeared that his back went out, causing very sharp pain to lower back.

Ee stepped in what he thought was a solid surface, which was, in fact, not solid. Overcompensation for balance lost caused ee to jerk backwards thereby spraining back muscles and spraining/straining Left knee and leg.

Employee was burning, grinding and welding inside a ball mill. He thought he had experienced a welding flash during this work session. The following evening he was so uncomfortable he went to an emergency room where it was learned that he had a piece of metal in his eye and it was removed.

Employee was attempting unhook chain from mill so it could be lifted away by a forklift. The forklift operator was instructed to raise the forklift blades and while doing so made contact with the accelerator, the forklift moved forward pinning employee between elevator chute and the forklift body. Employee suffered severe brusing and temporary mobility impairment.

Ee was lifting drive sprockets and drive chain to top of elevator with rope. Shoulder popped while lifting. This employee complained of this injury in January 2006, but was seen and released at that time. This is a revisit for that injury which required no treatment.

A rock wedged in slef-cleaning tail pulley of picking belt - Secondary Crusher. Stuck 3' long chipping hammer steel into pulley to try to dislodge rock. Pulley grabbed the steel and pulled ee's hand down and crushed it between the steel and bearing. Ee stepped on emergency pull cord and the belt stopped.

WHILE ATTEMPTING TO DISLODGE A JAM IN A BAGGING SPOUT, HEXAGIONAL ROD BECAME LODGED IN THE IMPELLER WHICH CAUSED FORCEFUL TWIST OF ROD. THE EE'SGLOVE AND PALM OF HIS RIGHT HAND WERE LACERATED BY THE FORCE OF THE TWIST.

EE'S THUMB WAS CAUGHT UNDER THE STRAP OF A SEMI-BULK BAG OF MATERIAL WHEN C0-WORKER ALLOWED THE BAG TO DESCEND PRIOR TO INSTRUCTIONS FROM BAGGER. THE FORCE OF THE BAG CAUSED THE INJURY.
