Showing all 9Employee was performing the daily servicing tasks (fueling & greasing) on the Kamatsu PC 750 excavator located in the bottom of the pit. While exiting the machine employee lost his footing and fell approximately 3 feet to the ground.

EMPLOYEE WAS DRIVING 769 OUT OF PIT. TRUCK BEGAN TO SMOKE AND THEN ENGINE STALLED. TRUCK ROLLED DOWN HILL, STEERED INTO HIGHWALL AND IT ROLLED OVER. EMPLOYEE WAS NOT INJURED. TRUCK CAUGHT ON F IRE, TRUCK TOTALED.
AT THE ENDING OF THE DAY WHILE GATHERING TOOLS, EE WAS STANDING ON A CONCRETE FOUNDATION AT THE SECONDARY CRUSHER WHICH WAS APPROX 14" HIGHER THAN THE GROUND ELEVATION AND LOST HIS BALANCE STE PPING BACKWARDS ONTO UNEVEN GROUND LANDING HEEL FIRST.
EE WAS PULLING ON PRYBAR TO TIGHTEN V-BELTS ON CRUSHER. BAR SLIPPED STRIKING EE IN NOSE. EE REPORTED HE WAS ALRIGHT AND CONTINUED TO WORK. DID NOT MISS ANY WORK. EE CONTACTED PLANT MANAGER AT HOME 8-27 TO REPORT HE HAD BEEN HURTING AND WENT TO DOCTOR. DOCTOR SAID HIS NOSE WAS BROKEN.
