While attempting to remove a seized bearing from a shaft, the employee struck the bearing with a hammer. Upon impact, a chip from the bearing struck the employee under their safety glasses.

The employee was working on the platform level of 471-BF-07. EE was raising cages by a rope to the upper section of the dust collector. EE looked up to communicate with the another employee. EE stepped backwards while continuing to look up and tripped over an empty box. When the employee braced EE's fall with EE's hands causing the injury to EE's right wrist.

A fire was observed at the northeast corner of the clinker cooler. Oil that leaked from the hydraulic system was ignited by hot material (approximately 1500F) that came from an undetected hole in the wall of the clinker cooler

The employee was entering the production utility shed outside of the administration building. When EE attempted to open the door the hinge failed and the door struck EE in the head causing a laceration.

The employee was changing a unplugged drill bit. EE was using EEs hands to loosen and tighten the drill bit in place. EE could not get a sufficient grip on the drill due to wearing gloves that had oil/grease on them as did the drill. EE removed EEs gloves to attempt to improve EEs grip. EEs hand slipped while tightening the drill and struck the tip of the bit causing a laceration.

The employee was installing blades on a shredder in the resource recovery area. They slid a blade onto the shaft and pinched their finger between the blade and the spacer.

A member of the office cleaning company was cleaning a restroom in the administration building. EE mixed two chemicals in the restroom which caused EE to inhale fumes. The vendor employee was taken to the emergency room and released later that day.

The employee was lowering a coil of hose from on top of a silo to the ground using rope. the weight of the hose caused the employee to lose grip on the rope momentarily. When the employee regained grip on the rope, they hit their right thumb on a guardrail causing the injury.

The employee was laying on grating reaching to install a bolt on the bearing housing, while a Johnson bar was leaning on a nearby handrail. Another employee leaned against the handrail which caused the Johnson bar to tip over and strike the employee on the head.

The CLG truck driver was attempting to dump a bulk load of ash at Giant Cement. The unit turned over while at the 3rd stage level. The driver was evaluated and issued prescription medication.

At the completion of a job on the Bypass Dust Collector the Production Coordinator was ensuring all valves for the system were secure. As they walked to the discharge valve on the bypass compartment (which was closed) a blast of dust came out from the Benetech inspection door. The material hit the employee in the face and head causing an injury to the left eye.

While walking up the stairs to the 3rd bank silo loadout. The employee was carrying two plastic bags in left hand and had backpack over right shoulder while on the way to the control room to start shift. The employee tripped on a stair tread and reached right arm out to brace the fall.

While tightening a tubing fitting for a pressure transmitter on the preheater tower, an air cannon in the area where the employee was working activated. This caused hot gas/air to exit through a small leak in a cardox port causing injury to the employees face.

While opening the quarry gate by pulling the gate open and walking along side of the gate; a rock became lodged under the gate causing a sudden stop of the gate's motion. The employee's arm/shoulder was pulled back suddenly causing a pain in the shoulder.

While walking from the parking lot to the building to begin working, the employee tripped over an uneven seam in the concrete parking lot. This caused EE to lose balance and fall to the ground landing on their hands and knees.

At approximately 4:oopm the employee was installing guards on the 471-DB-04 pan conveyor. While carrying a section of guard to the tail end for installation, the employee slipped on mud and fell to the ground with the guard section landing on EE's leg and back. The employee was issued a work restriction and prescription medication.

A fire was observed in the coal mill. It is likely that the slinger arms broke off in the coal mill equipment causing sparks and increased friction resulting in coal being heated and starting to combust.

While walking through the kiln/cooler area on 10/09/2023, the employee slipped on dust that had settled in the roadway causing EE's elbow to hit the pavement. On 10/10/2023 the employee requested a medical evaluation. On 10/24/2023 the employee had a MRI to evaluate the injury and that MRI revealed a fracture of bone and partial tear of tendon.

The employee was working on a kiln hydraulic pumps repair, and when EE was complete EE stood up and began to leave the area. When EE began walking, EE's boot caught on a hose that is attached to the equipment causing EE's to trip and hit EE's knee on the ground.

The employee was manually stacking 75LBS cement bags. After doing so, the employee recognized pain in lower back and leg. On May 15,2023 the employee completed the accident notification to the company. After a medical evaluation, the employee was returned to work with a restriction and having been prescribed medication.

The employee was raising a face shield which was attached to EE's hard hat. Dust had settled on the brim of the hard hat. When the employee raised the face shield, dust fell between the hard hat and the face shield landing behind EE's safety glasses into left eye. All dust was flushed out, the employee was issued prescription medication for inflammation.

The employee was loading blades onto a forklift. Once on the tine, the blade shifted forward smashing the employee's finger between that blade and a blade that had been previously loaded onto the forklift tine. On 11/22/22 the employee sought medical care and was diagnosed with a nondisplaced fracture of the left little finger

During the course of restarting finish mill 2 the employee was checking air slides for blockages. The employee opened an inspection door of an air slide and a blockage from upstream became dislodged. Dust from the dislodgement entered the employee's eyes although employee was wearing safety glasses.

The employee was using a cutting torch to remove corroded bolts from the synthetic gypsum system. After cutting 3 bolts the employee reoriented self to cut the 4th. This changed the direction that fumes would exhaust from the cutting. The employee felt dizzy and short of breath from the exposure and was issued prescription medication and restricted duty.

The employee was retrieving water from the warehouse to take to the pack house. While using a pocket knife to open the wrapping around the water bottles, the employee lacerated left index finger requiring the use of stitches.

The employee was near a screw conveyor on the 3rd floor of the finish mill 2 building. At some point their hand and arm became entangled in the screw conveyor. Emergency response was called immediately and the employee was transported to a hospital. The hospital later reported that the employee passed away.

On 04/07/22 the employee was closing a rail car door. On 04/18/22 the employee was evaluated by a occupational clinic and issued prescription medication. While doing so, a gear in the door slipped or the employee lost EE's grip of the metal bar EE was using as a tool to close the door with and EE's right knee struck the ground causing a contusion.

The employee was assigned to remove lids on a dust collector in finish mill 3. After removing the bolts that secure the lid in place the employee (along with a second employee) began to lift the lid. While lifting the lid, it became stuck in place. The employee attempted to quickly pull the lid out of the stuck position and strained a muscle in their chest.

While performing the task of removing aeration grates from the cooler floor they said they lifted a grate out of place and as they stood back up felt a pope in their lower back.

The employee was working on a blower on the preheater tower which was locked out prior to working on the equipment. The employee was using an air lance to dislodge a stuck blower. Employee attempted to push the blades with hand in order to loosen them. The blades began moving unexpectedly and at a high rate of speed pinching three fingers between the blades and the housing.

The employee was working at the coal mill to change the position of a cam roller; when EE stopped to take a break, EE stepped onto a 17-ton shackle. When EE stepped on the shackle EE rolled left ankle, causing a sprain.

The employee had hand pinched between a hydraulic cylinder and a base plate at the #4 roller in the raw mill. The employee was holding the cylinder (approximately 50 LBS) in place with one hand and cleaning the lower threaded section with other hand. The cylinder slipped and struck the employee in the hand that was cleaning the threaded section causing the laceration.

An employee was on knees reaching into a box attached to the FK pump at finish mill 2 to remove a flapper arm. When employee lifted the flapper arm employee felt a sudden pain in lower back. The employee was issued a work restriction and prescription medication.

The employee was entering the raw mill to supervise preventative maintenance. As EE opened the door, EE moved EE's hand from the handle and placed it on a bracket, which created a pinch point. EE's fingers were caught in the pinch point causing a laceration, contusion, and fracture of EE's fingers.

On 08/20/21 the employee working on grating knelt to perform assigned task. When repositioning body they twisted their knee in the grating rather than standing, turning and then kneeling a second time. On 08/23/21 the employee requested medical evaluation and was issued a work restriction.

All American Silo employees were walking pass feeder equipment when door on feeder was blown off. Employee took the blunt hit and others received little to no dust. Employee was taken to hospital for flushing of eyes after flushing them on site.

On 04/13/21 they informed the safety manager that they wanted a medical evaluation. The employee was issued a work restriction and prescription medication.

On 01/28/21 the employee was working to free a stuck cyclone in the vertical raw mill by using a shoulder to push against the equipment. In doing so the employee strained neck and shoulder and was issued a work restriction.

The conveyor, 471-DB-01, had been sprayed with used oil on 08/07/2020. On 08/08/2020 the kiln/cooler system became upset and caused unusually hot material to reach the conveyor. Once the hot material reached the conveyor, the oil that had been sprayed on the conveyor ignited. The fire was extinguished in approximately 35 minutes. There were no injuries associated with this event.
