While cleaning the clean air side of Silo 51 air slide, an employee found the slide choked and not flowing. EE used a short air lance to clear cement and restore flow. Cement blew back into EE's face, entering EE's right eye under EE's safety glasses. EE was sent to an eye doctor; no damage was found and antibiotic eye drops were prescribed. No lost time resulted. Returned to job.

On Feb 6, 2026, subcontractor, employee of Micronics (CRI MSHA ID C6220) was carrying blow pipes into the Keystone Cement Machine Shop. Leaving, their thumb was caught above the knuckle in a heavy counterweight-driven self-closing door that shut quickly and forcefully, causing a severe injury. The required stitches. The door's counterweight made it close fast, increasing the hazard

Miner was emptying dust collector, material flushed going in both eyes under glasses. Miner sent to emergency room, both eyes flushed out, eyes drops prescribe making this a reportable injury. Miner was released to full duty on 11/7/2025 after follow up appointment.

Miner was operating skid steer, backed into hole in roadway jarring the machine, EE's right elbow struck the safety bar and right shoulder contacted the cab enclosure causing a cervical strain and shoulder sprain. Injury became reportable on 10/7/2024 when employee received therapy for injury and restrictions.

Miner was installing bolt on frame of conveyor holding head of bolt with right hand. As miner ran nut to tighten with an impact driver, the washer on the head of the bolt pinched miner's right thumb tip against the conveyor frame causing laceration requiring stitches to close.

Employee was walking in stockpile area, slipped on ice covered with dust falling and striking right shoulder on ground. After continued pain in shoulder EE was sent for evaluation on 1/31/2022, MRI revealed torn rotator cuff in right shoulder. Injury became reportable on 2/18/2022 when surgery was performed to repair torn rotator cuff in right shoulder.

Miner was found lying on ground left side of 990H Loader, was seen minutes before on railed platform at rear of Loader. Miner was transported by ambulance to hospital, responsive at time of transport, trying to talk, attempting to sit up, remains in hospital for treatment for injuries.

Miner was walking on roadway at cooler, felt something go in right eye, was wearing all required PPE including safety glasses, had no issues with eye rest of work day, eye became irritated during the night, reported incident on 4/24/2018, sent to eye specialist, metal flake removed from right eye, written out of work for 4/24/2018 and 4/25/2018. Full duty 4/26/2018

Employee pulled water lance from port in tower, felt pain in right arm. Diagnosis was slight tear to bicep muscle, employee continued to fully perform his job duties until March 17 surgery, when this accident became reportable.

Employee was walking down steps on west side of cooler, he tripped on step and felt pain in his right knee, diagnosed with knee strain. Employee was able to fully perform job functions and returned to full duty. Pain continued in knee, sent for MRI. Employee was sent for surgery December 30, 2010, arthritis aggravated by trip on the step.

Employee was walking on roadway, stepped off roadway and twisted right knee. Employee was sent for evaluation, he was able to fully perform his job until 08/24/2010 when pain increased in knee and he was sent for MRI. Diagnosis was torn meniscus with no work restrictions placed on employee. This injury was deemed questionable and has now been deemed work related and reportable.

EE reported to supervisor on 1/19/2010 that his left knee was swollen, the employee did not report any accident, but felt it was due to excessive walking and stair climbing. On 01/25/2010 the employee was sent to Healthworks for evaluation, diagnosis was left hip and thigh strain due to excessive use. Employee was placed on restricted duty on 1/25/2010.

Employee was on the second step of a step platform 16 inches off the ground, he turned to lift down a section of 3 inch pipe that had been cut away from the main line. As he turned his right foot stepped on his left foot causing him to slip off the step, his left foot caught under the step twisting his left leg. He felt pain in his left shin, transported to hospital.

Employee was poking out a coal choke in the chute of a coal bunker, he pulled the bar he was using to remove the choke towards him and felt a sharp pain in his lower right back. Employee was transported to the hospital, he has a muscle strain and was placed on restricted duty.

Employee was walking through area at new kiln, when he twisted his left ankle and foot. Medical evaluation revealed a fractured bone in foot. A cast was applied and work restrictions mandated. The employee is a manager and is able to perform his job duties within the restrictions.

Employee was fueling the road sweeper, he stepped down off of platform at fuel tank and twisted right knee. Diagnosis by company doctor was stress fracture to right knee. This injury was contested by worker's comp. As of this date it has been verified as a work related injury.

Employee climbed onto platform at shrink wrap machine on the Packing Line to pull jammed plastic off of machine. As he stepped down from the platform to the floor he slipped and struck the floor with his left elbow. Injury sustained was a fractured left elbow.

Employee tripped on welding lead and twisted both knees, diagnosis-strain. Employee returned to work full duty, 11/26/2007 MRI showed meniscus tears in both knees. On 12/14/2007 employee had surgery on left knee. Injury had been contested as a pre-existing injury non work related. it became work related as of this date.

Employee was guiding excess chain from electric hoist around the crane drum, the weight of the chain caused excess chain to run out pulling employee's left hand into the railing. Left pinky finger was cut requiring stitches. Restricted duty only no lost time.

Employee stepped down off platform at Kiln 2 Vent Fan, twisted knee and felt pain. He reported incident but refused medical treatment on this date. Follow up showed torn left medial meniscas in left knee. Surgery on 11/02/2006 made this a reportable lost time accident as of this date.
