Employee was cutting a slab of rubber with a dirty box cutter at Separator 2 area when EE cut self "on the last bit (end)" Employee was taken to emergency room where EE received wound cleaning and closure with staples to EE's right thigh.

An operator, needed assistance getting a loader out of the shop. They had a 10ft stand under the lift arm, supporting the bucket. When I went to pull the stand out from under the loader, I injured my back. I felt a sharp pain in my lower back immediately. I laid down for a few minutes, then left work to go to the chiropractor.

EE illness is due to torching & burning metal while dismantling SEP01. EE did not report throat discomfort, coughing & congestion until 9/18/24. On 9/18/24 EE visited the ER & received treatment for acute bronchitis, chest x-rays were negative. Doctor prescribed medication & told employee to follow up with primary doctor. The EE was released back to work with no restrictions.

While using EE's pocketknife in EE's right hand to remove zip ties from a frame used to hold video sheet, EE accidentally slipped and cut EE's left hand near EE's thumb. EE received 1 stitch and a TDAT shot.

Working at the blasting operation, EE was walking from one down hole to another when EE stepped on a rock and felt their left knee move. EE was able to continue the shift and the knee started hurting the next day 04/27/22.

Employee stepped off a skiff onto the trailer of the skiff and lost balance, employee extended hands as they fell forward, they bent the right middle and ring finger backwards causing laceration at the top joint. Employee received sutures on both fingers.

EE was lifting a hydraulic rock splitter from a rock when EE felt EE's right shoulder give out. EE was asked if EE wanted to do another job, EE said "no" EE was alright. Monday 6/19/17 EE said EE's back felt like it was going out, at that time went to a less strenuous job. EE's on restricted duty not lifting more than 10lb, PT for 4 weeks and then re-evaluation.

EE was retrieving a rock splitter tool from a rock when the rock separated, causing the tool to move EE's hand against the side of the man basket and when the rock separated, the man basket moved causing EE to hit right knee on the side of the man basket. EE declined medical attention on 6/14/16, EE called in from a scheduled day off on 6/21/16 requesting to see a doctor.

EE was pulling barge line on stern on barge 43, he stated the barge was muddy and he slipped, feeling something wrong in his left knee, he had no pain right away but by the end of the day he felt it hurting. Medical attention 1/28/15, MRI in Torrance Ca on 1/31/15, limits no prolonged walking/climbing, he is still working, we will update report when results from MRI are received.

EE said he bent over to go under the sill leg at the S-60 when he heard a "pop" in his back, he sat down, said he'd be alright, minutes later he said he was going home to rest his back, left the Quarry at 10:47am. He called the Quarry at 11:20 in extreme pain he was at the C.I.M.C., a nurse took him into the Hospital, failed to file report before leaving on his own.

Removing shaft from S-60 Derrick Stiff Leg crane draw works. Shaft slid inside and at the same time shifting weight upwards. Injury either tried to hold the shaft and it pulled him in or he did not have time to get out of the way, smashing his left forearm between the shaft flange and draw works.

While grinding on a material screen plant a foreign object got into his right eye. He didn't notice it until 5:00 in the evening when showering. He could feel something in his eye and reported it the following day at work and went to the doctor and a piece or rust was removed, he has a corneal abraison.

No specific working condition caused employee to have a seizure, he has an existing condition that causes seizures. He was traveling within the Quarry when he became confused, a co-worker was with him at the time, and helped him until he became alert again.

A large slab of rock had lodged itself inside the hopper at Separator #2. Plant was shut down and employee was using a pry bar to dislodge it when the rock came free. Other rocks followed causing him to lose control of the pry bar. The bar smashed his right hand into the side of the hopper, injuring his right middle finger.

A drill steel got stuck when EE was drilling down holes. He used a sledgehammer to free the steel. Once the steel came out, he bent over to pick up some drill lubricant and his back went out.

EE CAME TO WORK 10/7/03 COMPLAINING OF BACK PAIN. HE WORKED 10/8/03, 10/9/03 & 1/2 DAY 10/10/03. HE WENT TO THE DOCTOR 10/13/03. NOT KNOWING WHAT CAUSED HIS PAIN HE PLANNED ON USING HIS UNION INSURANCE. THE DOCTOR RELEASED HIM BACK TO WORK WITH NO RESTRCITIONS ON 10/15/03.

HE LANDED ON HIS LEFT SIDE ON THE CLOSED TAILGATE OF THE PICK UP TRUCK. HE RECEIVED SEVERE CONTUSIONS AND MUSCULAR STRAIN TO HIS LEFT RIBS, SIDE AND BACK.
