EE was cleaning debris under the bottom conveyor of crusher, when EE step out away from crusher and watching rollers the step fell down towards head. Leaning down hardhat fell off. 2nd staff assisted the staff that was hit by step, secured the wound area with gauze as it was bleeding. 2nd staff got EE to a unit and rushed to ER for treatment. 15 min from pit to ER.

Showing all 5Unknown- According to Subject, started feeling sick walking on flat surface to slight down hill grade surface. A short time later subject reported to foreman they were feeling ill and wanted to go to the hospital. at that time a call was made for medical personnel to meet at the St. Johns Fire Dept. Once at the F.D. EMS transported subject to hospital.
Miner got out of equipment to smoke a cigarette, and when returning to backhoe he tripped over the out rigger and fell on his left shoulder
OPERATOR WAS WAITING FOR CRUSHER TO EMPTY TO GO TO LUNCH. HE GOT A PART OF BUCKET FOR TRACTION TO GO UP THE HILL TO PUSH MATERIAL, WHEN HE RAN OVER A PILE OF MATERIAL AND THE LOADER TIPPED OVER. (DRIVER WAS NOT INJURED.)
EMPLOYEE WAS STANDING IN THE HOPPER OF THE STACKER. WHILE ADJUSTING THE GATES ON THE SHAKER SCREEN, THE CONVEYOR BELT WAS ACCIDENTLY STARTED AND THE EMPLOYEE WAS KNOCKED OFF HIS FEET AND BUMPE D HIS HEAD AND SCRATCHED HIS RIGHT LEG.
