KOSMOS CEMENT CO. · Accident type, without injuries

An insulator on a 138kv electrical power pole went to ground and caused power outage and a unplanned fire on the pole at the north end of the plant. The fire burned 67 minutes until FD put it out. The pole is owned by the utility provider, not the mine.

On Friday Dec. 13th the entire plant lost electrical power at 11:01am. There were two employees in the old raw mill elevator when the power went out. They became entrapped in the elevator between the 3rd and 4th floor. Radio contact was maintained & both employees were in good health. Plant regained power at 11:37 and elevator went to landing & employees got out.

Production Supervisor was cleaning out material from a choked screw conveyor with hand. The Supervisor's hand ran across the sharp flight of the screw and caused a laceration which required 4 stitches to close the wound. Screw was locked out at time of incident.

A production process attendant injured EE's left middle finger while pushing in a pin on the chute above the clinker cooler drag conveyor. When pushing in the pin to break up the chunk, the chunk cleared causing EE's forward momentum to strike EE's hand between the pin and the chute. This resulted in a laceration and fracture of the left middle finger.

A maintenance repair person was tasked with replacing a transfer pump on the grinding aid tank. The employee stepped back, and EE's right leg entered an opening in the grating. The employee was also experiencing pain in the groin area when lifting. After seeking medical treatment, the employee was diagnosed with a hernia. The employee had surgery on November 25, 2019.

On June 15, 2019, Employee stepped on a golf ball sized rock on the barge. The employee was attempting to secure a barge when they stepped on rock causing them to roll their ankle. On July 2 2019, a physician prescribed PT and a walking boot.

An employee pinched a finger between a bearing housing and the pedestal base. Two employees were installing the tail drum for a small-scale belt. The injured employee was positioning a base bolt from underneath the pedestal and had their finger slightly under the load when it was let off causing a laceration of the skin. Received 5 stitches and restricted duty.

Three employees from a contracting company became entrapped in elevator. There was contact with the employees and they were in good health. The elevator maintenance contractor arrived and rescued the miners at 12:42.

Employee was clearing an air slide on top of the cement silo when the dust collector flushed and cement dust hit the employee in the face. Cement dust entered the employees eyes, employee received a prescription for cornea abrasion.

Employee was using a vac truck to clean the cone of a cement silo when material came out. Cement entered the employee's right eye. The employee was taken to the eye doctor and was prescribed eye drops.

Employee was climbing fixed ladder to go to the 914 drag conveyor. The employee's foot slipped off of the ladder rung. The employee caught self with right hand, the employee felt pain in right shoulder. Employee was seen by the doctor, diagnosed with a shoulder strain and prescribed physical therapy.

Shipper/packer/loader was attempting to lift a barge winch cable off of the barge when ee felt a pain in lower back. Employee declined medical attention at that time. On May 3, employee requested to be seen by doctor. Employee was diagnosed with lower back strain and prescribed physical therapy.

Employee was working on coal mill tightening bolts on Thursday 2/15/2018. Employee states employee felt sore. Employee reported the soreness on Friday 16th and wished to seek medical attention. The employee was diagnosed with strains of both shoulders and placed on restricted duty.

Mobile equipment mechanic was attempting to fill hydraulic oil in a bull dozer. The EE was standing on the bed of the service truck when EE turned to the left. The employee accidentally stepped off of the bed fell to the ground. The employee was diagnosed with a wrist strain/sprain and an elbow contusion. EE was given a brace for the wrist and was placed on restricted duty.

Two production employees were attempting to empty the pre-hopper of the fine coal bin. When they opened the gate, nothing came out. When they closed the gate, the employees felt a percussion, heard a bang and the room filled with coal dust. The employees exited the room uninjured.

Maintenance Mechanic was attempting to loosen a 1" water pipe union. The employee was pushing on a pipe wrench with left arm when employee felt a pain in shoulder. The employee did not wish seek medical attention at that time. On January 4, 2018, the employee was seen by a doctor, and was placed on restricted duty.

Employee was attempting to clear debris around a yoke in order to align a pin slot in the secondary crusher. At the same time another employee dislodged a rock on the other side of the yoke and the crusher apron dropped. The employees left thumb was crushed between the yoke and the crusher apron, causing a chip of the bone and tissue damage to the employee's left thumb.

Employee was installing a guard back on coupling. The employee lost control of the guard and it slid over and pinched left ring finger between the guard and side of motor. This caused a laceration requiring 2 stitches and a fracture of the tip of the finger.

On 6/17/17 Employee was swapping compressed gas cylinders when employee lost balance and was struck by cylinder on the lower leg. Employee wound was treated contusion and abrasion with first aid. On 7/6/17, employee was still experiencing pain and wanted it evaluated. The employee's wound had gotten infected, employee given Rx for antibiotics

Employee was using a 5 lb sledge hammer and poke bar to clean material build up in the tower. While striking the poke bar with the hammer, the hammer head slipped off of the head of the poke bar and the employees right point finger was crushed between the handle of the hammer and the head of the poke bar. The finger tip was fractured and stitches were used to close the wound.

EE was walking on an access platform of a mill ball sorter. The ee missed a step and caught self with left arm to avoid falling. The ee felt a pain in left shoulder. Employee sought medical attention the day of injury and was released to full duty. After a follow up evaluation with a orthopedic on 5/8/2017, the employee was placed on restricted duty.

During warm up of the kiln, there was an unplanned ignition of fuel. The ignition destroyed the cooler roof. Two employees were in the area, but no injuries occurred as a result of the ignition. The cause of the ignition has been determined as an excessive amount of unburned fuel in the kiln. The flame on the burner went out to drafting issues in the system.

EMPLOYEE WAS USING TORCH TO DEMO STEEL LINERS OUT OF CHUTE. ONE OF THE LINERS WAS UNKNOWINGLY LOOSE AND WHILE PRYING ON THE LINER THE EMPLOYEE HAD JUST FINISHED CUTTING, THE LOOSE LINER ON THE ADJACENT WALL TIPPED OVER SMASHING THE EMPLOYEE'S FINGER BETWEEN THE TWO.

Employee was lifting a mill liner that weighs approx 65 lbs when the liner slipped from employee's grip and pinched the left ring finger causing a laceration. When in the process a wet condition caused the gloves to be wet. Employee was taken to a local hospital and received stitches to close the lacerations. X-rays showed no fractures.

Employee stated ee wanted to go to a doctor on 8/16 after getting struck on the side of hard hat with a cable while wearing it. EE claimed it occurred on 8/9 but waited 7 days to go see a dr to get checked out after suffering headaches and insomnia.

Employee was changing a cable pulley that raised and lowered a load spout. While installing the new pulley, something broke. The cable struck the employee in the head area fatally injuring the employee. The investigation is still ongoing to determine the exact cause.

Mechanical Repairman was cutting up scrap metal when they attempted to squat and lift an 8' piece and felt a burning sensation in abdomen/groin area. EE ended up re-aggravating a prior hernia injury from 2014.

A process attendant received minor irritation to the eyes when hydrated lime inadvertently flushed out of a port when a cap fell off. The employee was attempting to unblock a chute by using a threaded pipe when the cap was accidently hit and came off. Prescribed eye drops were given to the employee for the prevention of infection.

4 contractor employees with Mineral Fabrication were traveling to the 6th floor of the preheat tower when they lost power to the car. The employee immediately called plant control room and Murphy Elevator techs were notified. The Elevator Governor Breaker was reset and employees exited elevator at 9:52 AM. Employees were not injured nor were they in any danger at any time.

Portable electric die grinder lost power momentarily and once it kicked back on the tool fell out of employee's hand with the grinding wheel cutting the top of left hand by ring finger. Very poor design on this tool and all have been taken out of service for our Company.

Laborer while walking around a corner in the shop bumped into an repairman while ee was trimming a poster with a utility knife. The accidental contact caused the left hand of the laborer to become lacerated by the index finger requiring 5 stitches.

A process attendant was in the preheat elevator traveling to the 10th floor when the elevator car stopped between the 6th and 9th floor. Process attendant became entrapped in elevator at 2:40 PM and exited at 4:00 PM. Elevator technicians arrived at plant to troubleshoot and adjust the governor in the machine room. There were no injuries as a result of this incident.
